Abstract
Analysis of the original documents on Hunter's operation revealed that Hunter himself never realized the theoretical implications of his operation, particularly that collateral circulation could develop following ligation of the main arterial trunk of a limb. This operation, however, marks a monumental step in the history of vascular surgery. Even though supplanted only one century later by Matas' obliterating endoaneurysmorraphy, this technique became the first efficient means of managing popliteal aneurysms, the prognosis of which had been absolutely disastrous until that time, leading to either amputation or death in most cases.
Resumen
Tratamiento de los aneurismas antes de 1785: El método tradicional consistía en la ligadura inmediatamente por encima y por debajo de la zona dilatada con el vaciamiento del saco aneurismático. La única variación de importancia es atribuída a Anel, consistente en la ligadura sólo por encima del aneurisma sin abrir el saco. Las ligaduras se efectuaban en bloque con materiales de sutura que eran expulsados tras dos semanas de infección y en ocasiones produciéndose hemorragias secundarias. El mismo Hunter en este período era partidario, cuando el aneurisma era de pequeño calibre de vaciamiento del aneurisma precedido de ligadura arterial, preconizaba la amputación en estadíos avanzados de la enfermedad.La nueva concepción: la operación de Hunter: El 12 de Diciembre de 1785, John Hunter trató con éxito a un paciente con aneurisma de arteria poplítea con su nueva técnica de ligadura de femoral superficial en zona sana. Esta ligadura se lleva a cabo utilizando cuatro suturas «para evitar una excesiva presión sobre el vaso en una zona determinada». En cuatro sucesivas ocasiones en que empleó esta técnica tuvo éxito en tres. Los principios que se derivaban de esta técnica fueron principalmente dos: El primero referente a la circulación colateral, «creo que la circulación colateral siempre se desarrolló después de la ligadura de la arteria femoral superficial». Es de justicia sin embargo constatar, que el fenómeno de la circulación colateral ya era conocido por otros autores. El segundo principio, consistía en la asunción de que el aneurisma era una angiopatía localizada y que las hemorragias mortales eran consecuencia de la afectación de la arteria adyacente. Con la ligadura, se interrumpía la presión sobre el saco aneurismtico y se evitaban estas hemorragias.Impacto y vigencia de la operación de Hunter: Al efectuar esta técnica Hunter rompe con las enseñanzas que habían perdurado durante más de 200 años. Lo que es más importante al extrapolar los principios de esta intervención y aplicarlos a otros problemas vasculares, se dio nuevo ímpetu a la cirugía arterial. A pesar de que fue un hito en la cirugía la vigencia de esta intervención no fue superior a los 100 años. Ya en casos particulares se podía predecir el fallo de esta técnica como eran situaciones de inflamación aneurismática, insuficiente circulación colateral y degeneración arterial. Fue sobre todo la técnica de endoaneurismorrafia de Rudolph Matas el que hizo declinar la operación de Hunter. Posteriormente, con la era antiséptica se consiguió la resección del saco aneurismático con resultados superiores a la operación descrita por John Hunter el 12 de Diciembre de 1785.
