A statistical comparison between different tests for the diagnosis of latent diabetes and their clinical relevance

Summary

It is not yet clear which of the well known tolerance tests for carbohydrate metabolism should be preferred. In order to contribute to the solution of this question, we tried out and compared in 89 selected patients of our out-patient department the following 4 methods: 1) OGTT (50 g glucose); 2) OGTT (100 g glucose); 3) IVGTT; 4) TT. Using various criteria described in the literature, the results where evaluated and compared statistically (correlation coefficient, fourfold table). Higly significant correlations have been found when comparing the IVGTT with the TT. Though OGTTs showed a good correlation with each other, the correlation coefficient was less than 0.5 in comparison with the two i.v. tests (IVGTT and TT). The discussion shows which of the methods should be preferred for the diagnosis of the diabetic disturbance of carbohydrate metabolism, the ‘physiological’ or the i.v. tests. Some methods of evaluation are set out which lead to a better accordance between the results of the 4 tests.
