Bundle nailing for treatment of humeral fractures

Abstract

Surgical Principles

Bundle nailing is based on the principle of elastic intramedullary tight fixation. This can only be achieved by a correct technique and by meeting the four essential provisions:

  1. 1.

    “Fensterschnürung”, by which Hackethal described the jamming of the nail ends in the small cortical window, through which the nails are inserted.

  2. 2.

    “Taillenschnürung”, this means the jamming of nails in the isthmus of the shaft.

  3. 3.

    “Spongiosanagelung” describes the impaction of the proximal nail tips into the metaphyseal bone.

  4. 4.

    “Verkeilungsnägel” (short nails) are used to fill the proximal metaphyseal cavity.

Bundle nailing can be performed from proximal or distal, either way three to five nails should pass the fracture site.
