Zusammenfassung
Grundlagen: Die fokale noduläre Hyperplasie (FNH) ist eine seltene benigne Lebererkrankung. Durch die modernen bildgebenden Verfahren wird sie gehäuft als Zufallsbefund diagnostiziert. Die MRT und die Hepatocholeszintigraphie haben einen hohen Stellenwert bei der Diagnosestellung. Eine sichere Diagnose kann nur durch die histologische Untersuchung gestellt werden.
Methodik: In den 25 Jahren (1969 bis 1994) wurden 46 Frauen und 2 Männer mit einer FNH behandelt und 1 bis 25 Jahre lang verfolgt.
Ergebnisse: Die FNH war bei 11 Patientinnen größer als 10 cm, 3 davon mit intratumoraler Blutung. Alle diese Patientinnen hatten mehr als 6 Jahre lang Kontrazeptiva eingenommen. Bei je 1 Patientin wurde gleichzeitig ein Leberzelladenom und ein Leberzellkarzinom festgestellt. Eine Resektion wurde bei 36 Patienten und Probeexzision bzw. Probepunktion bei 12 Patienten durchgeführt, ohne Morbidität und Mortalität. Ein Rezidiv wurde nicht beobachtet. Eine spontane Regression innerhalb von 12 Jahren konnte bei 1 Patientin computertomographisch nachgewiesen werden.
Schlußfolgerungen: Die asymptomatische FNH bedarf keiner operativen Behandlung, und eine kontrollierte Weiterbeobachtung ist gerechtfertigt. Eine Operation ist nur bei symptomatischer FNH indiziert.
Summary
Background: The focal nodular hyperplasia (FNH) is an uncommon benign and mostly asymptomatic hepatic lesion. Increasing FNH were diagnosed incidentally by the modern radiological images. The MRT and the scintigraphy are of special value in the preoperative diagnosis. However a definite diagnosis can only be achieved by histological examination.
Methods: In the past 25 years (1969 to 1994) we have treated and followed up 1 to 25 years 46 female and 2 male patients with FNH.
Results: The FNH was greater than 10 cm in 11 patients, of whom 3 had intratumoral bleeding. All of these 11 patients had taken contraceptives more than 6 years. Liver cell adenoma was found in 1 and carcinoma in another patient. Liver resection was carried out in 36 patients, probe excision or biopsy in 12 patients without morbidity and mortality. No recurrence has been observed. Spontaneous regression within 12 years was documented by CT in 1 patient.
Conclusions: Asymptomatic FNH needs no surgical treatment and observation is justified. Indication for operation is given by symptomatic FNH only.
