Ten years of experience in 152 patients after modified fontan procedures

Abstract

From 1980 to 1990 152 patients underwent Fontan operation at our institution. The following patient groups amenable for Fontan operation were identified: 1) patients with tricuspid atresia (n=82, 54.0%); 2) patients with single ventricle (n=31,20.3%), 3) patients with a wide variety of “complex”, non correctable cardiac malformations (n=39, 25.7%). Actuarial survival rate was 83.8±3.1% (mean±SEM) at 10 years. Follow up revealed, that more than 90.0% of the patients feel better than before operation, about 50.0% are without cyanosis, more than 70.0% are in sinus rhythm, and 90.0% of the patients are socialized according to their age. Considering the poor prognosis of these patients without an operation, the Fontan procedure offers a sufficient palliative method in the therapeutic approach up to now.

Zusammenfassung

Von 1980 bis 1990 wurde bei 152 Patienten die Indikation zu einer Fontan-Operation in unserer Klinik gesehen. Bei 82 (54,0%) Patienten war eine Trikuspidalklappenatresie diagnostiziert, bei 31 (20,3%) Patienten ein singulärer Ventrikel und bei 39 (25,7%) Patienten ein komplexer, nicht korrigierbarer Herzfehler. Die 10-Jahres-Überlebensrate beträgt 83,8±3,1% (Mittelwert±SEM). Eine Nachuntersuchung der Patienten ergab, daß mehr als 90,0% der Patienten sich besser als vor der Fontan-Operation fühlen, beinahe 50,0% der Patienten sind ohne Zeichen einer Zyanose, mehr als 70,0% der Patienten haben Sinusrhythmus und mehr als 90,0% der Patienten sind entsprechend ihrem Alter schulisch und beruflich integriert. Berücksichtigt man die ungünstige Prognose der Patienten ohne chirurgische Therapie bietet sich die Fontanoperation als eine gute palliative Methode für Patienten mit ansonsten nicht behandelbaren kardialen Mißbildungen an.
