Summary
One hundred twenty-seven artificially ventilated patients with acute respiratory insufficiency (ARI) were investigated. In 61 patients positive end-expiratory pressure (PEEP) was used when ventilation with ZEEP proved to be insufficient for one or more of the following reasons: increasing I-aDO2m PaO2 below 60 Torr at FiO2≧0.5, detoriation of clinical status (group=secondary PEEP). The time elapse between beginning of artificial ventilation and institution of PEEP was 46±47 h (median 33 h). In 66 patients PEEP was used from the beginning of artificial ventilation (group II=primary PEEP). The distribution of underlying diseases (severe poisoning, pancreatitis, polytrauma or major surgery, pneumonia, cardiovascular failure, sepsis) as well as the frequency of additional vital function failure (circulatory shock, acute renal failure) were comparable in both groups (p>0.05). At the beginning of artificial ventilation both groups were comparable in respect to respiratory insufficiency. PaO2 was 75±26 Torr in group I and 70±29 Torr in group II at comparable levels of FiO2 (p>0.05). PaCO2 was 34.7±8.2 Torr in group I and 37.4±10.5 Torr in group II. Significantly more patients in group II received corticosteroids (>1 gr/die). Mortality was 48/61 (79%) in group I and 37/66 (56%) in group II (p<0.01). End-inspiratory pressure exceeding 35 cm H2O was necessary in 42/61 patients in group I and 28/66 patients in group II (p<0.01) and FiO2>0.5 was necessary to keep PaO2 above 60 Torr in 39/61 patients in group I and 27/66 patients in group II (p<0.01). It is concluted that early institution of PEEP improves the course and outcome of patients with ARI.
Zusammenfassung
Bei 127 Patienten mit beatmungsbedürftiger akuter respiratorischer Insuffizienz wurde der Zusammenhang zwischen Zeitpunkt des Einsatzes der PEEP Beatmung und Behandlungergebnis untersucht. Bei 61 Patienten wurde in konventioneller Weise zunächst mit ZEEP beatmet und auf PEEP übergegangen, wenn die inspiratorisch-arterielle O2-Differenz sich vergrößerte, PaO2 bei FiO2≧0,5 unter 60 mm Hg lag, die radiologischen Lungenveränderungen zunahmen, oder eine Verschlechterung des Allgemeinzustandes offenkundig war (Gruppe I=sekundärer PEEP, zeitliche Latenz zwischen Intubation und PEEP im Mittel 46±47 Std., Medianwert 33 h). 66 Patienten wurden bei gegebener Indikation zur Beatmung unabhängig von diesen Kriterien sofort mit PEEP beatmet (Gruppe II=primärer PEEP). Beide Gruppen waren vergleichbar hinsichtlich der Grunddiagnosen, der Häufigkeit von Schock und akutem Nierenversagen, der Vorbehandlung in auswärtigen Kliniken sowie des Schweregrades der respiratorischen Insuffizienz bei Behandlungsbeginn (PaO2 Gr. I 75±26 mm Hg, Gr. II 70±29 mm Hg,p>0,05, bei vergleichbarer O2 Beimischungen zum Zeitpunkt der Messung. PaCO2-Gr. I 34.7±8.2 mm Hg, Gr. II 37.4±10,5 mm Hg,p>0,05). Die Patienten beider Gruppen erhielten gleichhäufig Heparin, jedoch wurden in Gruppe II signifikant häufiger Steroide in hoher Dosis gegeben. Die Letalität war in Gr. II (primärer PEEP) mit 37/66 Pat. (65%) signifikant niedriger als in Gr. I (sekundärer PEEP) mit 48/61 Pat. (79%) (p<0,01). Respiratorische Parameter zeigten in Gr. II ein günstigeres Verhalten im Verlauf (FiO2>0,5 bei 27/66 Pat. in Gr. II, bei 39/61 Pat. in Gr. I,p<0,01, ΔAMV>10% über Ausgangswert bei 21/66 Pat. in Gr. II, bei 40/61 Pat. in Gr. I,p<0,01, EIP>35 cm H2O bei 28/66 Pat. in Gr. II bei 42/61 Pat. in Gr. I,p<0,01). Primärer PEEP verbessert die Prognose bei akuter respiratorischer Insuffizienz.
