Abstract
Young male rats were treated with one of three diphosphonates, disodium ethane-1-hydroxy-1,1-diphosphonate (EHDP), disodium dichloromethylene-diphosphonate (Cl2MDP), or disodium methane-1-cyclohexyl-1-hydroxy-1,1-diphosphonate (MCHDP), or with porcine calcitonin. Changes in the length weight, calcium, phosphorus, and ash of the femora and in plasma calcium, phosphate and alkaline phosphatase were measured. Bone formation rate was measured histologically in the tibial diaphysis using tetracycline markers. The lower dose (0.01–1mg P/kg/day s.c.) of the phosphonates, which block immobilization osteoporosis and reduce bone turnover measured by45Ca kinetics, did not cause marked changes in the composition of the femora or in plasma values. This suggests that, at these doses in intact animals, the diphosphonates similarly reduce both resorption and mineralization rates. Higher doses of EHDP and MCHDP (10 or 30 mg P/kg/day s.c.) impaired bone growth both in length and width and inhibited mineralization of new cartilage and osteoid. The total calcium content of the bones decreased but the plasma values remained unchanged. Cl2MDP did not impair mineralization at equivalent doses. EHDP, unlike Cl2MDP, therefore seems to have two effects on bone. At lower doses it reduces bone turnover rate; but at higher doses it also directly prevents the full mineralization of new matrix. This difference between the effects of EHDP and Cl2MDP may be important.
Résumé
Des jeunes rats mâles (70–90g) ont été traités pendant dix jours avec 1) l’un des trois diphosphonates suivants: l’éthane-1-hydroxy-1,1-diphosphonate disodique (EHDP), le dichlorométhylène diphosphonate disodique (Cl2MDP) ou le méthane-1-cyclohéxyl-1-hydroxy-1,1-diphosphonate disodique (MCHDP), 2) de la calcitonine porcine. L’influence de ces traitements sur la longueur le poids, le contenu en calcium, phosphore et cendres des fémurs, ainsi que sur le calcium, le phosphate et la phosphatase alcaline du plasma a été évaluée. La vitesse de la formation osseuse a été mesurée histologiquement dans la diaphyse du tibia en utilisant le marquage à la tétracycline. Des études précédentes ont montré qu’aux faibles doses (0,01–1 mg P/kg/jour s.c.), les diphosphonates peuvent bloquer l’ostéoporose d’immobilisation et réduire le remodellement osseux mesuré par la cinétique du Ca45. Dans cette étude, ces doses ne produisent pas d’altération de l’os fémoral et des valeurs plasmatiques. Ces résultats indiquent que, chez des animaux intacts, ces faibles doses de diphosphonate diminuent les vitesses de résorption et de minéralisation osseuses au même degré. Des doses élevées d’EHDP et de MCHDP (10 ou 30 mg P/kg/jour s.c.) réduisent la croissance osseuse dans sa longueur et sa largeur et inhibent la minéralisation du cartilage et de la substance ostéoïde récemment déposés. Le contenu total en calcium de l’os diminue, mais les valeurs plasmatiques ne changent pas. A des doses correspondantes, le Cl2MDP n’interfère pas avec le processus de minéralisation. L’EHDP, contrairement au Cl2MDP, paraît donc avoir deux effets sur l’os. A faibles doses, il réduit la vitesse du remodellement osseux; mais à doses élevées, il inhibe aussi directement la minéralisation de la matrice osseuse récemment déposée. Cette différence entre les effets de l’EHDP et du Cl2MDP pourrait être d’importance lors de leur utilisation thérapeutique.
Zusammenfassung
Junge männliche Ratten (70–90 g) wurden während 10 Tagen mit einem von drei Diphosphonaten—Dinatrium Äthan-1-hydroxy-1,1-Diphosphonat (EHDP), Dinatrium Dichloromethylen-Diphosphonat (Cl2MDP) oder Dinatrium Methan-1-cyclohexyl-1-hydroxy-1,1-Diphosphonat (MCHDP)—oder mit Schweine-Calcitonin behandelt. Die Veränderungen in Länge und Gewicht und in Calcium-, Phosphor- und Aschgehalt der Femora wurden gemessen, sowie Veränderungen in Plasma Calcium, Phosphat und alkalischer Phosphatase. Die Knochenbildungsrate wurde in der Tibia-Diaphyse mit Hilfe von Tetracyclin-Markierung histologisch gemessen. Frühere Untersuchungen hatten gezeigt, daß die niedrigeren Dosen (0,01–1 mg P/kg/Tag s.c.) der Phosphonate die Immobilisations-Osteoporose blockierten und den mit45Ca-Kinetik gemessenen Knochenumbau herabsetzten. Diese Dosen bewirkten in dieser Studie keine deutlichen Veränderungen in der Zusammensetzung der Femora oder in den Plasmawerten. Dies läßt vermuten, daß niedrigere Dosen von Diphosphonaten bei intakten Tieren die Resorptions- und Mineralisationsraten in gleichem Maße herabsetzen. Höhere Dosen von EHDP und MCHDP (10 oder 30 mg P/kg/Tag s.c.) beeinträchtigten das Knochenwachstum in der Länge und in der Breite und hemmten die Mineralisation von frisch abgelagertem Knorpel und Osteoid. Der Gesamtcalciumgehalt der Knochen verminderte sich, aber die Plasmawerte waren unverändert. Hingegen beeinträchtigten gleiche Dosen von Cl2MDP die Mineralisation nicht. EHDP scheint also zwei Wirkungen auf den Knochen auszuüben. Bei niedrigeren Dosen vermindert es die Knochenumbaurate; bei höheren Dosen verhindert es jedoch auch direkt die volle Mineralisation von frischer Knochenmatrix, was bei Cl2MDP nicht der Fall ist. Diese unterschiedlichen Wirkungen von EHDP und Cl2MDP könnten für die Therapie wichtig sein.
