Abstract
In 1962, Chanceet al. demonstrated the feasibility of monitoring intramitochondrial NADH levelsin vivo. To extend this capability to the neurosurgical operating room on a practical basis, and to enable a comparison of metabolic changes in two different cortical areas a dual-beam long-focal-length fluorometer for the direct measurement of NADH fluorescence changes was designed.
A 300 mm working distance minimises the requirements for sterilisation and reduces interference to normal operating procedures. The technique consists of illuminating the cortex by a commutated mercury-arc-lamp beam with the resulting fluorescence monitored by a photomultiplier tube system. Switching the ultraviolet excitation sequentially from a beam-off reference to each of the target spots on the cortex improves the stability and reduces interference from ambient lighting. Variations in fluorescence are related to metabolic changes which result from electrical stimulation or spontaneous epileptiform activity.

En 1962, Chanceet al. démontrèrent qu'il est possible de contrôler les niveaux NADH intramitochrondrauxin vivo. Afin d'étendre cette possibilité aux salles d'opérations neurochirurgicales de façon pratique et de permettre de comparer les changements métaboliques dans deux différentes zones corticales, un fluorimètre à deux faisceaux et à distance focale longue a été conçu pour effectuer la mesure directe des variations de fluorescence NADH.
Une distance opérationelle de 300 mm réduit au minimum les besoins de stérilisation et évite l'encombrement pendant les procédés chirurgicaux. La technique consiste à illuminer le cortex à l'aide d'une lampe commutée à arc de mercure, la fluorescence résultante étant contrôlée à l'aide d'un système à tube photomultiplicateur. La stabilité est améliorée et l'interférence par la lumière ambiante est réduite en faisant passer l'excitation ultraviolette en séquence de la position référence avec le faisceau éteint à chacun des points déterminés du cortex.
Les variations de fluorescence dépendent des changements métaboliques résultant de la stimulation électrique ou de l'activité épileptiforme spontanée.

1962 zeigtenChance et al., daß es möglich ist, die intramitochondrischen NADH-Wertein vivo zu überwachen. Um diese Möglichkeit auf den neuro-chirurgischen Operationsraum auf praktischer Grundlage auszudehnen und um einen Vergleich der Änderungen im Sauerstoffaustausch in zwei verschiedenen kortikalen Bereichen durchführen zu können, wurde ein Doppelstrahl-Fluorometer mit langer Brennweite zur direkten Messung der Änderungen in der NADH-Fluoreszenz konstruiert.
Durch eine Arbeitsentfernung von 300 mm werden die an die Sterilisation gestellten Bedingungen auf ein Mindestmaß beschränkt, und die Interferenz wird auf normale Betriebsverfahren reduziert. Das Verfahren besteht daraus, daß die Kortex durch einen kommutierten Quecksilberdampf-Lampenstrahl beleuchtet wird, wobei die sich ergebende Fluoreszenz von einem Fotovervielfacher-Röhrensystem überwacht wird. Durch Schalten der ultravioletten Erregung in der richtigen Reihenfolge von einem Strahl außerhalf des Bezugspunktes auf jeden der Zielpunkte der Kortex verbessert sich die Stabilität, und die Störung durch Raumbeleuchtung wird gesenkt. Schwankungen in der Fluoreszenz werden auf metabolische Schwankungen bezogen, die sich aus der elektrischen Stimulation oder aus spontaner, epileptiformer Tätigkeit ergeben.
