Abstract
A new perfusion and transducer unit for ambulatory blood-pressure recording is described, followed by reports of both the laboratory evaluation of the unit and experience with its clinical use. The major change from earlier systems is the perfusion system, now based on an electrically driven ‘delta’ pump. Pressure is measured by a semiconductor strain-gauge pressure transducer. Laboratory evaluation has shown that the transducer with associated tubing and cannulae has a satisfactory frequency response, considerably better than that of the recording system normally used. Both quantitatively and qualitatively there was a very close correspondence between signals obtained from the entire Oxford system (including recording and replay) and signals obtained directly by means of a catheter-tip transducer. Small, but consistent, signal drifts attributable to battery behaviour are described. The system appears remarkably insensitive to temperature variations within the range of expected ambient temperatures. Delta pump performance is satisfactory, producing essentially constant perfusion rates. Clinically, the system has been used for the recording of ambulatory blood pressures in 110 patients for periods up to 72 h with satisfactory results. Reliability appears good, with minimal artefacts atributable to the equipment. This transducer and perfusion system appears to be fully satisfactory and considerably more reliable than the earlier designs.

Une nouvelle unité de perfusion à transducteur pour l'enregistrement de la pression sanguine d'un malade ambulatoire est décrite et les rapports sur l'évaluation en laboratoire et l'expérimentation clinique de l'unité sont présentés. Le changement majeur par rapport au système précédent est le dispositif de perfusion, basé maintenant sur une pompe Delta électrique. La pression est mesurée par un transducteur à manocontact à déformation à semiconducteur. L'évaluation en laboratoire a montré que le transducteur avec son tubage et sa cannule associés a une courbe de fréquences satisfaisante et nettement supérieure à celle des systèmes d'enregistrement normalement utilisés. Du point de vue quantitatif comme qualitatif, on a observé une correspondance très étroite entre les signaux fournis par le système Oxford complet (y compris enregistrement et écoute) et ceux obtenus directement par un transducteur à l'extrémité d'un cathéter. Les variations de signal petites mais constantes atribuables aux piles sont décrites. Le système paraît remarquablement peu sensible aux variations de température dans les gammes de températures ambiantes courantes. Les performances de la pompe Delta sont satisfaisantes, ce qui permet d'avoir des taux de perssions sanguines de Du point de vue clinique, le système a été utilisé pour l'enregistrement des pressions sanguines de 110 malades ambulatoires pendant des périodes allant jusqu'à 72 heures, et ce avec des résultats satisfaisants. La fiabilité paraît bonne, avec un minimum de problèmes atribuables au matériel. Ce transducteur avec son système de perfusion paraît très satisfaisant et beaucoup plus fiable que les dispositifs précédents.
Zusammenfassung
Es wird ein neues Perfusions- und Wandlergerät zur Aufzeichnung des ambulatorischen Blutdrucks beschrieben und anschließend wird auf Berichte über die Laborauswertung des Geräts und die im klinischen Gebrauch gemachten Erfahrungen eingegangen. Das Gerät unterscheidet sich von frflheren Systemen im wesentlichen durch das Perfusionssystem, das jetzt auf einer elektrisch betriebenen Deltapumpe beruht. Der Druck wird mit Hilfe eines Halbleiter-Spannungsmesser-Druckwandlers gemessen. Die Laborauswertung hat gezeigt, daß der Wandler mit den dazugehörigen Schläuchen und Kannülen einen befriedigenden Frequenzgang aufweist, der viel besser ist als bei dem normalerweise angewandten System. Sowohl quantitativ als auch qualitativ bestand ein sehr enger Zusammenhang zwischen den vom Oxford-System (Aufzeichnung und Wiedergabe) empfangenen Signalen und direkt über einen Katheterspitzenwandler empfangenen Signalen. Es werden kleine, aber einheitliche Signaldrifts beschrieben, die auf das Verhalten der Batterie zurückzuführen sind. Das System scheint gegenüber Temperaturschwankungen innerhalb des Bereichs der zu erwartenden Umgebungstemperatur erstaunlich unempfindlich zu sein. Die Deltapumpenleistung ist befriedigend und erzeugt eine im wesentlichen konstante Perfusionsrate. Klinisch wurde das System zur Aufzeichnung des ambulatorischen Blutdrucks bei 110 Patienten für eine Zeit von bis zu 72 Stunden mit befriedigenden Ergebnissen eingesetzt. Die Zuverlässigkeit scheint gut zu sein, wobei geringfügige Artefakte auf die Ausrüstung zurückzuführen sind. Dieses Wandler-und Perfusionssystem scheint vollständig befriedigend und weit zuverlässiger zu sein, als die früheren Ausführungen.
