Blutgasanalytische Untersuchungen während homologer Lebertransplantationen am Hausschwein

Changes of the Acid-Base-Balance were studied in 42 pigs which received orthotopically homologous liver transplants. The studies were carried out during the transplantation procedure by means of blood gas analysis (micro-method of Astrup). Acidoses were developed in 35 experiments. Their nature was respiratory in the initial phase of liver transplantation. During the anhepatic stage and following revascularisation of the graft the acidosis was predominantly metabolic with corresponding elevation of serum lactic acid levels. The reasons for these changes were mostly organic and could be explained by the transplantation technique. The continuous control of the blood gas analytic values during experimental and clinical liver transplantation is of great importance for the outcome of the operation.

Zusammenfassung

Bei 42 orthotopen homologen Leberverpflanzungen am Schwein wurden die Veränderungen des Säure-Basen-Haushaltes während der Transplantation mittels Blutgasanalyse untersucht. Bei 35 Versuchen kamen Acidosen zur Beobachtung. Diese waren in der Initialphase der Lebertransplantation vorwiegend respiratorisch, in der anhepatischen Phase und nach Revascularisation des Transplantates vorwiegend metabolisch, mit entsprechender Anhebung der Lactatspiegel. Die auslösenden Ursachen waren meist organisch und durch die Transplantationsmethode bedingt. Eine entsprechende gezielte Puffertherapie ist durch eine ständige Kontrolle der blutgasanalytischen Werte möglich, die für ein positives Ergebnis nach einer experimentellen oder klinischen Lebertransplantation unerläßlich erscheint.
