Examination of circulatory regulation during orthostatic adaptation phase

Part 1: Evaluation of rapid alterations of heart volume. A roentgenological method

Summary

A simple roentgenological procedure is described, by which it is possible to measure the heart volume and its alteration during early orthostatic adaptation.

Long distance x-ray films were taken in the postero-anterior projection using a 70-mm camera with a frequency of 2 to 6 exposures per second. The length and width of the heart were determined from the films and the plane of the heart calculated with consideration of the parallaxis, existent in the geometry of the system, the overall magnifying factor and the distortion, which is present at the rim of the image intensifier. The mean horizontal depth diameter, that is necessary to calculate the volume of the heart could be determined by indirect means. In every test-person the depth diameter belonging to the heart plane was determined by aid of the individual relation between the different planes of the heart and the respective depth diameters.

Comparisons between the volumes determined roentgenologically and the true values revealed a difference of less than 3%.

There is no statistically significant difference in the mean values between the heart volumes, that have been determined in 12 test persons by the method described, and the commonly used method. The alterations of the heart volume in 10 testpersons were investigated during acute orthostatic stress. It could be shown that the heart volume, which was 693 L±24,8 ml before the orthostatic stress, decreased within 6,9±1,6 sec statistically significant by 16,5%.

The analysis of the time course of the heart volume alterations during orthostatic stress showed:

    The alterations of the heart volume, which were observed during orthostatic adaptation were not caused by a lowered diaphragm.

  2. 2.

    No statistically significant correlation could be found between the alterations of the heart volume and the respective respiration phase.

  3. 3.

    Statistically significant differences between heart volumes, found in enddiastolic and endsystolic phases, did not correspond to the sum of the stroke volume of the left and right ventricle. However it was found, that alterations of the stroke volume could be determined qualitatively by the procedure described.
