Summary
29 livers of pig were preserved by an intermittent perfusion (by gravidity) in hypothermia. The perfusate was enriched with potassium and glucose.
Independent from the perfusion pressure all recipients survived longer than 4 days, if the preservation time did not exceed more than 8 hrs.
When the preservation pressure was 50–60 cm H2O and the preservation time 12–13 hrs, only 2 of 6 animals survived the orthotopic transplantation longer than 4 days. If we used as preservation pressure 10–15 cm H2O, then all recipients survived 12–13 hrs preservation longer than 6 days.
By admixture of so-called cell stabilisators (magnesium, prednisolone, chlorpromazine) the ischaemic tolerance time could not be extended. The substitution of glucose by xylit in the perfusate did not make any effect. The morphological signs of a rejection appeared in all animals from the 5th post operative day and showed an extraordinary variation in regard of its intensity. In general the longer preserved livers showed a higher degree of rejection.
In all livers, however, morphologically infectious complications were to be seen (so-called hepatic sepsis).
Zusammenfassung
29 Schweinelebern wurden mittels einer intermittierenden Perfusion in Hypothermie konserviert. Das Perfusat war mit Kalium und Glucose angereichert. Unabhängig vom Perfusionsdruck überlebten alle Empfänger mehr als 4 Tage, sofern die Konservierung 8 Std nicht überschritt. Bei einem Perfusionsdruck von 50–60 cm H2O und 12–13stündiger Konservierung überlebten nur 2 von 6 Tieren die orthotope Transplantation mehr als 4 Tage. Wurde der Perfusionsdruck jedoch auf 15–20 cm H2O gesenkt, überlebten alle Empfänger bei gleicher Konservierungsdauer mehr als 6 Tage. Durch Zusatz von sog. Zellstabilisatoren (Mg++, Prednisolon, Chlorpromazin) konnte die Ischämietoleranzzeit nicht verlängert werden. Der Ersatz von Glucose durch Xylit im Perfusat blieb ebenfalls ohne Effekt. Die morphologischen Zeichen einer Abstoßung traten bei fast allen Tieren ab dem 5. Tag der Transplantation auf und zeigten eine außerordentliche Variationsbreite bezüglich des Ausmaßes und des zeitlichen Ablaufs. Im allgemeinen war die Abstoßungsreaktion bei länger konservierten Lebern stärker ausgeprägt.
