24. Frühe Komplikationen bei der Oesophagus-Carcinom-Chirurgie mit intrathorakalen Anastomosen

Summary

From 1974–1984 137 patients with esophageal carcinoma have been operated using intrathoracal anastomoses. From 1974–1978 (58 patients) the early complication rate was 65%, clinical letality was 32% in regard to different surgical procedures. Since 1979 79 patients were resected by means of an abdominal-right thoracotomy approach in one session employing a standardised pre- and postoperative therapeutic regime. The complication rate thus could be reduced from 64% to 37%, the clinical lethality from 32% to 16% in regard to nearly the same patient population. The complications of both periods are compared in relation to the surgical procedures and to the possibilities of reintervention.

Zusammenfassung

Von 1974–1984 wurden 137 Oesophagus-Carcinom-Patienten mit intrathorakalen Anastomosen versorgt. Von 1974–1978 (58 Patienten) lag, bei Anwendung differierender Op.-Methoden, die Frühkomplikationsrate bei 64%, die Klinikletalität bei 32%. Seit 1979 wurde bei 79 Patienten einzeitig abdomino-rechtsthorakal reseziert mit standardisiertem prä- u. postoperativem Behandlungsregime. Bei etwa gleicher Qualität des Krankengutes konnte dadurch die Komplikationsrate insgesamt von 64% auf 37% gesenkt werden, die Klinikletalität von 32% auf 16%. Die Komplikationen aus beiden Perioden werden anhand der Operationsverfahren und der Reinterventionsmöglichkeiten gegenübergestellt.
