Clostridium perfringens septicemia with massive hemolysis

Summary

Massive hemolysis and renal failure are rare complications of infection withClostridium perfringens, resulting in a very high mortality rate (70–100%). The severity of the infection depends on the presence of underlying conditions such as malignancies and diabetes mellitus. In patients without underlying disorders, massive hemolysis and anuria have been observed in only eight cases, according to recent reports. This case report describes a 61-year-old man who died ofC. perfringens septicemia with massive hemolysis and anuria less than 4 h after admittance to the hospital, despite rapid and adequate antibiotic treatment. No focal infection was found.

Zusammenfassung

Massive Hämolyse und Nierenversagen treten nur selten als Komplikation einer Infektion durchClostridium perfringens auf; die Letalität ist mit 70 bis 100% extrem hoch. Maligne Erkrankungen und Diabetes mellitus oder andere Grundkrankheiten sind wichtige Determinanten für den Schweregrad der Infektion. Nach neueren Berichten wurden bei Patienten ohne Grundkrankheit nur acht Fälle von Gasbrand mit massiver Hämolyse und Anurie beobachtet. Im vorliegenden Fall entwickelte ein 61 Jahre alter Mann eineC. perfringens-Septikämie mit massiver Hämolyse und Anurie. Der Tod trat innerhalb von weniger als vier Stunden nach Krankenhauseinweisung ein, obwohl rasch eine adäquate Antibiotikatherapie eingeleitet wurde. Ein Infektionsherd war nicht zu finden.
