Abstract
From 1951 through December, 1985, a total of 89 patients underwent total pancreatectomy for ductal adenocarcinoma. Of the 89 patients, 54 were male and 35 were female; their average age was 60.0 years. Histologic grading (Broders') demonstrated that 56% of the patients had a grade 2 tumor and 38%, a grade 3 tumor. None of the patients had grade 4 tumors. Peripancreatic lymph nodes were positive in 47% of the patients at the time of resection. Multicentricity was noted in 32.5% of patients. The operative mortality was 10.1%; however, for the patients who had surgery after January, 1980, the operative mortality was 6.2%. At the completion of the study, 78 patients had died and 11 were alive. The principal cause of death was recurrent malignancy. The median duration of survival, including the duration of survival in patients who had operative deaths, was 1.03 years. The influence of sex, tumor grade and size, multicentricity, and nodal status was evaluated multivariately with Cox's proportional hazard model.

Entre 1951 et décembre, 1985, 89 patients ont subi une pancréatectomie totale pour adénocarcinome canalaire du pancréas. Il y avait 54 hommes et 35 femmes; l'âge moyen était de 60.0 ans. Selon la classification histologique (de Broders), 6% avaient une tumeur de classe I, 56% avaient une tumeur de classe II, et 38% une tumeur de classe III. Aucun patient n'avait de tumeur de classe IV. Les ganglions lymphatiques péripancréatiques étaient envahis chez 47% des patients au moment de la résection. La tumeur était multicentrique chez 32.5% des patients. La mortalité opératoire était de 10.1%; cependant, cette mortalité n'est que de 6.2% depuis janvier, 1980. A la fin de cette étude, 78 patients étaient morts, 11 étaient vivants. La cause principale de mort était la reprise du processus malin. La durée médiane de survie était de 1.03 ans. Entre 1951 y diciembre de 1985 un total de 89 pacientes fue sometido a pancreatectomía total por adenocarcinoma ductal, 54 hombres y 35 mujeres, con edad promedio de 60.0 anos. La gradación histológica de Broder demostró grado tumoral 2 en 56% y grado 3 en 38%; ninguno exhibió grado 4. Los ganglios linfáticos peripancreáticos aparecieron positivos en 47% de los pacientes en el momento de la resección y se encontró multicentricidad en 32.5%. La mortalidad operatoria fue 10.1%, pero en los pacientes operados después de enero de 1980, la mortalidad fue 6.2%. Al término del estudio 78 pacientes habían muerto y 11 se encontraban vivos. La principal causa de muerte fue recurrencia de la neoplasia. La duración media de la supervivencia, incluyendo la supervivencia de los pacientes con mortalidad operatoria, fue 1.03 años.
