Controlled trials in the treatment of “early” breast cancer: A review of published results

Abstract

This review summarizes the results of local treatment of “early” breast cancer obtained in 17 clinical trials that can be divided into 3 groups. In the 7 trials in group I, the type of surgery in each was a constant (radical mastectomy or simple mastectomy) and the evaluations were designed to compare the results of various forms of postoperative treatment (immediate radiotherapy, delayed radiotherapy, chemotherapy, placebo, and observation). In the 4 trials in group II, the postoperative management in each was a constant (radiotherapy or observation) and the variable that was evaluated was the type of surgery (simple mastectomy versus radical mastectomy). In the 6 trials in group III, both the type of surgery (super-radical mastectomy, radical mastectomy, simple mastectomy, wide local excision) and the type of postoperative treatment (partial or complete radiotherapy, observation) were varied so as to evaluate different combinations. Although sufficient time has not elapsed to assess the results of many of these trials, the conclusions that appear warranted at this time are: 1) simple mastectomy combined with postoperative radiotherapy is as effective in terms of survival and recurrence rates as radical mastectomy with or without postoperative radiotherapy in stage I and stage II breast cancer, and 2) postoperative radiotherapy does not increase survival following radical mastectomy.

Résumé

Cette revue résume les résultats du traitement local du cancer du sein “débutant” dans 17 essais cliniques qui peuvent Être divisés en 3 groupes. Dans les 7 essais du groupe I, le type de chirurgie était constant pour chacun d'eux (mastectomie radicale ou mastectomie simple) et l'on a évalué les résultats des diverses formes de traitement postopératoire (radiothérapie immédiate, radiothérapie différée, chimiothérapie, placebo, observation). Dans les 4 essais du groupe II, le traitement postopératoire était constant (soit radiothérapie, soit observation) et la variable à évaluer était l'intervention chirurgicale (mastectomie simple ou mastectomie radicale). Dans les 6 essais du groupe III, le type de chirurgie (mastectomie super-radicale, radicale ou simple, excision locale large) et le type de traitement postopératoire (radiothérapie complète ou partielle, observation, ovariectomie) ont varié de faÇon à pouvoir évaluer diverses combinaisons. Bien que la période d'observation soit trop courte pour permettre une appréciation correcte de la plupart des essais, on peut déjà tirer les conclusions suivantes: 1) la mastectomie simple combinée à la radiothérapie postopératoire est aussi efficace que la mastectomie radicale avec ou sans radiothérapie postopératoire, en termes de survie et de fréquence des récidives dans les cancers du sein au stades I et II, 2) la radiothérapie postopératoire n'accroÎt pas la survie après mastectomie radicale.
