Summary
The intestinal59Fe-absorption from the diagnostic 0.56 mg59Fe2+-dose was measured with a 4π geometry whole body radioactivity detector with liquid organic scintillator in patients with gastric mucosa atrophy and after partial or total gastrectomy. The diffuse cytoplasmic hemosiderin storage iron of the bone marrow reticuloendothelial cells was stained by Berlin blue reaction and classified into normo-, hypo-, and hypersiderosis.
All the patients withgastric mucosa atrophy and absolute intrinsic factor deficiency (vitamin B12 treated pernicious anemia in remission) showed normal amounts of diffuse storage iron in their bone marrow macrophages and absorbed59Fe2+ within the normal range of 7–54% (\(\bar X\) a ±S \(\bar x\) a =27±3.2%). Normal iron stores were observed in about 50% of the patients withpartial gastrectomy (Billroth I or II). All these patients absorbed59Fe2+ within the normal range (5–60%;\(\bar X\) a ±S \(\bar x\) a =32±2.7%). The other 50% of partially gastrectomized patients had depleted iron stores. Without any exception, a considerably increased59Fe absorption was measured in the patients with prelatent (\(\bar X\) a ±S \(\bar x\) a =68±4.0%), latent (79±4.8%) or manifest (84±3.0) iron deficiency. The high negative correlation between the diffuse cytoplasmic storage iron of the bone marrow macrophages and the results of the diagnostic 0.56 mg59Fe2+-absorption test which was described earlier for subjects with a normal gastric mucosa does therefore also exist in patients with gastric mucosa atrophy and absolute intrinsic factor deficiency as well as in patients having undergone partial gastrectomy. Normal gastric juice or a gastric intrinsic factor are not required for normal ferrous iron absorption. The 0.56 mg59Fe2+ absorption test can be used for the diagnosis of depleted iron stores also in patients with gastric mucosa atrophy or partial gastrectomy.
Total gastrectomy reduced the59Fe-absorption from the 0.56 mg59Fe2+-dose to 2.8–12% (\(\bar X\) a ±S \(\bar x\) a =6.5±1.6%) or one fourth of the normal value although all these patients had normal or even elevated amounts of diffuse storage iron in their bone marrow macrophages.
Zusammenfassung
Bei Patienten mit Magenmucosaatrophie und absolutem Intrinsic Factor-Mangel sowie nach Magen-2/3-Resektion bzw. totaler Gastrektomie wurde die59Fe-Resorption aus der diagnostischen 0,56 mg59Fe2+-Testdosis mit dem Großraum-Radioaktivitätsdetektor gemessen und das im Cytoplasma der Knochenmarksmakrophagen diffus verteilte Hämosiderin-Reserveeisen mit der Berliner-Blau-Reaktion dargestellt.
Alle Patienten mitMagenmucosaatrophie enthielten normale Mengen an diffus verteiltem Reserveeisen und resorbierten entsprechend mit 27±3,2% (\(\bar X\) a ±S \(\bar x\) a ) normale59Fe2+-Mengen. Bei denjenigen nach Billroth I oder II operierten Patienten mitMagen-2/3-Resektion, bei denen noch normale Mengen an diffusem Reserveeisen nachgewiesen worden waren, wurde ebenfalls eine mit 5–60% (\(\bar X\) a ±S \(\bar x\) a =32±2,7%) im Normalbereich liegende diagnostische59Fe2+-Resorption festgestellt. Eine stark heraufgesetzte diagnostische59Fe2+-Resorption (Bereich 50–97%;\(\bar X\) a ±S \(\bar x\) a =68±4,0 bis 84±3,0) ließ sich bei allen magenresezierten Patienten mit erschöpften Eisenreserven und prälatentem, latentem bzw. manifestem Eisenmangel nachweisen. Es besteht somit nicht nur bei magengesunden Personen, sondern auch bei Patienten mit Magenmucosaatrophie bzw. Magen-2/3-Resektion eine negative Korrelation zwischen den eytochemisch darstellbaren Mengen an diffus verteiltem und utilisierbarem Reserveeisen und der durch Gesamtkörperretentionsmessung bestimmbaren diagnostischen59Fe2+-Resorption. Für die normale Ferroeisen-Resorption ist somit weder die Magensalzsäure noch ein gastrischer „Intrinsic Factor“ erforderlich. Der diagnostische59Fe2+-Resorptions-Gesamtkörperretentions-Test kann auch bei Patienten mit Magenmucosaatrophie bzw. Magen-2/3-Resektion für die Erkennung normaler oder erschöpfter Eisenreserven eingesetzt werden.
Totalgastrektomierte Patienten enthielten zwar normale bis vermehrte Mengen an diffusem Reserveeisen, resorbierten aber nur 2,8–12% (\(\bar X\) a ±S \(\bar x\) a =6,5±1,6%) der diagnostischen59Fe2+-Testdosis.
