Summary
The conversion of cholesterol to bile acids was measured in 6 patients with essential hypercholesterolemia (type II hyperlipoproteinemia), 9 patients with endogenous hypertriglyceridemia (type IV hyperlipoproteinemia) and one patient with symptomatic hypertriglyceridemia by using14C-labeled cholic acid and3H-labeled chenodeoxycholic acid. The total daily turnover of primary bile acids in patients with type II- hyperlipoproteinemia was 278±80 mg/day. This value is lower than the mean value found in normal subjects. The turnover in patients with type IV hyperlipoproteinemia was 1139±306 mg/day, i.e. an approx. four-fold increase. It is concluded that differences in cholesterol metabolism exist in different types of hyperlipoproteinemia and that no conclusions can be drawn in regard to cholesterol catabolism from plasma cholesterol levels.
Zusammenfassung
Bei 6 Personen mit essentieller Hypercholesterinämie (Typ II-Hyperlipoproteinämie), 9 Personen mit primärer endogener Hypertriglyceridämie (Typ IV-Hyper-lipoproteinämie) und einem Fall von symptomatischer Hypertriglyceridämie wurde der Abbau von Cholesterin zu Gallensäuren bestimmt mit Hilfe14C-markierter Cholsäure und3H-markierter Chenodesoxycholsäure. Bei 6 Personen mit Typ II Hyperlipoproteinämie betrug die Syntheserate der Gallensäuren 278±80 mg/Tag. Dieser Wert liegt niedriger als der bei Normalpersonen erhobene Durchschnittswert. Die Syntheserate betrug bei 9 Personen mit Typ IV-Hyperlipoproteinämie 1 139±306 mg/Tag und war damit etwa um das Vierfache gesteigert gegenüber der Vergleichsgruppe. Eine Patientin mit einer sekundären Hypertriglyceridämie bei Diabetes mellitus und Übergewicht zeigte eine normale Abbaurate des Cholesterins zu Gallensäuren. Aus den Befunden folgt, daß bei den verschiedenen Typen der Hyperlipoproteinämien Unterschiede bezüglich des Cholesterinstoffwechsels bestehen und aus der Höhe des Plasmacholesterinspiegels keine Rückschlüsse auf die Abbaurate des Cholesterins zu Gallensäuren möglich sind.
