Summary
In an attempt to investigate the clinical importance of a dextranase present in human tissues, a number of patient suffering from latent or manifest Diabetes mellitus were subjected to infusions with Rheomacrodex and Macrodex. These infusions did not cause any variations in the diurnal blood sugar curve and in the excretion of glucose in the urine (collected over 24 hours), compared to normal controls. The results seemed to indicate that even in cases of a reduced tolerance for glucose the hydrolytic metabolism of dextrans is of little quantitative importance in the human organism. The results also showed that even repeated infusions of dextran do not induce a clinically significant activation or induction of the dextranase within 10 days. The investigation showed that there is no reason to restrict the administration of dextrans in patients with Diabetes mellitus.
Zusammenfassung
Zur Frage der klinischen Bedeutung einer in menschlichen Geweben nachgewiesenen Dextranase wurden bei Pat. mit latentem und manifestem Diabetes mellitus Rheomacrodex- und Macrodexinfusionen durchgeführt. Gegenüber den Normalbedingungen konnten unter den Dextraninfusionen Änderungen der Blutzuckertageskurven und der Glucoseausscheidung im 24-Std-Urin nicht festgestellt werden. Zusammengefaßt sprechen die Ergebnisse dafür, daß auch beim Vorliegen einer herabgesetzten Glucosetoleranz der hydrolytische Abbau der Dextrane im menschlichen Organismus quantitativ zu vernachlässigen ist. Den Befunden entsprechend tritt auch eine klinisch bedeutsame Aktivierung oder Induktion der Dextranase unter wiederholter Dextraninfusion bis zu 10 Tagen nicht ein. Aus den Untersuchungen ergibt sich keine Einschränkung für die Verabreichung von Dextranen bei Pat. mit einem Diabetes mellitus.
