Also at the age of infant and children disturbances of the intestinal circulation are playing an important role in the pathogenesis of different diseases or their complications.
Prenatal disturbances of the blood circulation of the gut heal in form of jejunoileal atresias and stenosis. These cases show under what circumstances the most grave disturbances of the circulation of the intestine could be treated in theory under the following circumstances: 1. sterile intestine and 2. prevention of shock.
Postnatal disturbances of the intestinal circulation may have different origins. Primary occlusion of mesenteric are seldom in childhood, however, single cases are published time and again. Much more often, occlusion or of their branches occurs through strangulations (volvulus, intussusception, incarceration). At first, the danger for the patient does not consist in a gangrene and in a resulting general peritonitis, but in the shock reaction caused by the occlusion of the mesenteric vassels. After a few hours of ischemia the gut is still viable. The disturbances of microcirculation and vasoconstriction, especially in the mesenteric vessels are of essential importance in these events. These disturbances of the blood circulation of the gut are not only brought about by shock caused by occlusion of the mesenteric vessels, but also occur as a result of vasoconstriction by shock of any other origin or also by shock of toxic basis. This quite peripheral disturbance of the circulation, localized especially in the mucous membrane, becomes the ruling symptom and reduces the original cause (trauma, ileus, infection etc.) to the background. In this situation the most important management is the interruption of vasoconstriction and the influencing of microcirculation.
The question of how to further the recovery of the bowel after temporary ischaemia, gains great importance above all in the case of total and subtotal infarction of the gut as a consequence of volvulus. In these cases, often lost until now,Rehbein has suggested a two-stage operation with primary removal of the volvulus and resection of the gangrenous gut 24 to 48 h later. This question is examinated in this paper also in experiments on animals. It appears that remedies, which are used also in the therapy of shock (dexamethason, adenosinephosphates, phenoxybenzamin) favourably influence the recovery of the gut. The best resultats are obtained with medicines, which interrupt vasoconstrietion. On the one hand a primary recovery of the bowel could be ascertained, on the other, a short time after the damage a rapid regeneration of the mucous membrane could be seen.

Intestinale Zirkulationsstörungen spielen auch im Säuglings-und Kindesalter eine bedeutende Rolle in der Pathogenese verschiedener Erkrankungen oder ihrer Komplikationen.
Pränatal aufgetretene Durchblutungsstörungen des Darmes führen zu Defektheilungen in Form vonAtresien undStenosen des Jejunums und Ileums. Diese Fälle zeigen, unter welchen Bedingungen schwerste Durchblutungsstörungen des Darmes theoretisch heilen können, nämlich unter folgenden Voraussetzungen: 1. steriler Darmtrakt und 2. Verhinderung eines Schockzustandes.
Postnatale intestinale Zirkulationsstörungen können durch verschiedene Ursachen entstehen. Primäre Verschlüsse der Mesenterialgefäße sind im Kindesalter selten, einzelne Fälle werden aber immer wieder mitgeteilt. Wesentlich häufiger kommt es im Kindesalter zum Verschluß der Mesenterialgefäße oder ihrer Äste durch Strangulation (Volvulus, Invagination, Incarceration). Die Gefahr für den Patienten bestcht zunächst nicht in einer Darmgangrän und einer dadurchhhervorgerufenen Perforationsperitonitis, sondern in den durch den Gefäßverschluß ausgelöstenSchockreaktionen. Während der Darm noch nach mehrstündiger Ischämie lebensfähig bleibt, bedroht der Shock das Leben des Patienten. Im Zentrum des Geschehens stehen die Störungen der Mikrozirkulation und die Vasoconstriction, besonders im Bereiche der Darmgefäße.
Zu dieser Durchblutungsstörung des Darmes kommt es aber nicht nur durch den von einem Mesenterialgefäßverschluß ausgelösten Schockzustand, sondern auf Grund einerVasoconstriction beim Schock jeder anderen Genese oder auchauf toxischer Basis. Diese ganz periphere, besonders in der Darmschleimhaut lokalisierte Zirkulationsstörung wird zum beherrschenden Krankheitsbild und läßt die auslösende Ursache (Trauma, Ileus, Infektion usw.) in den Hintergrund treten. Die wesentlichste therapeutische Maßnahme in dieser Situation ist dieDurchbrechung der Vasoconstriction und die Beeinflussung der Mikrozirkulation.
Die Frage, wie die Erholung des Darmes nach temporärer Ischämie gefördert werden könne, gewinnt vor allem bei totaler oder subtotaler Infarzierung des Darmes infolge Volvulus große Bedeutung. Bei diesen bisher häufig verlorenen Fällen hatRehbein ein zweizeitiges Vorgehen mit primärer Beseitigung des Volvulus und Resektion des gangränösen Darmes 24 bis 48 Std später angeregt. Diese Frage wurde in der vorliegenden Arbeit auch in Tierexperimenten untersucht. Es zeigte sich, daß Mittel, die auch in der Schocktherapic Anwendung finden (Dexamethason, Adenosinphosphate, Phenoxybenzamin), die Darmerholung günstig beeinflussen. Die besten Resultate wurden mit Medikamenten erzielt, die die Vasoconstriction unterbrechen. Einerseits konnte eine primäre Erholung des Darmes festgestellt werden, andererseits aber auch schon kurze Zeit nach der Schädigung eine lebhafte Regeneration der Schleimhaut gesehen werden.
