Summary
From 1952 to 1967 1418 patients of the Evangelisches Krankenhaus Göttingen-Weende were operated on the biliary tract. The results of this study have been interpreted as follows:

There was no increase of operations on the biliary tract compared to the total number of operations.

2.
The number of carcinoma of the duodenal papilla increased, the number of carcinoma of the gallbladder remained the same.

3.
The maximum of all operations on the biliary tract was performed on patients between 50 and 59 years of age; the maximum of all cancer operations on the biliary tract on patients between 60 and 69 years of age. Patients less than 40 years old did not have carcinoma of the biliary tract.

The values of the individual carcinoma groups of the biliary tract varied according to their location:

a)
The carcinoma of the gallbladder was found only on women; the carcinoma of the duodenal papilla was found only one and a half times more on women than on men.

b)
More metastases of the liver were found when the carcinoma was located near the liver: gallbladder-carcinoma 15%, carcinoma at the porta hepatis 14%. Metastases of the liver were found in the carcinoma of the common bile tract and in the duodenal papilla in 0% and 8% of the cases respectively.

c)
The most frequent histologic type of carcinoma of the biliary tract was the adenocarcinoma followed by the solid carcinoma. This applies more (3.5 times as much) to the gallbladder-carcinoma than to the other carcinoma types of the biliary tract.

d)
The largest group of patients with a gallbladder-carcinoma had complaints for 7 to 12 months, the largest group of patients with another carcinoma type at the biliary tract had complaints for 2 to 6 months. Much longer case histories occurred as well.

e)
Gallstone colics occurred more often with gallbladder- and porta hepatis carcinoma than with common bile tract- and duodenal papilla carcinoma.
The increase of biliary tract carcinoma in recent years in our study depends nearly exclusively on the increase of duodenal papilla carcinoma while the number of gallbladder-carcinoma remained the same. But is is often difficult to differentiate the duodenal papilla carcinoma from the carcinoma of the head of the pancreas. Furthermore carcinome of the duodenal papilla does not depend only on influences from the biliary tract region. Therefore the reports about an increase of carcinoma of the biliary tract must be regarded with reserve. Without differentiating between the tumor location they might lead to wrong conclusions.
It does not seem logical to regard the carcinoma of the biliary tract as a unity, although one is always again temptated to combine all the different carcinoma groups of the biliary tract due to their rarity. It is advisable to present seperately the carcinoma types of the biliary tract which are differentiated by their location in order to get a basis for collective statistics of informative value.
Zusammenfassung
In der Zeit zwischen 1952 und 1967 wurden im Evangelischen Krankenhaus Göttingen-Weende 1418 Pat. am Gallenwegssystem operiert. Die Aufzeichnungen hierüber wurden in dieser Arbeit ausgewertet und brachten folgende Ergebnisse:

Es fand sich keine Zunahme der Operationen am Gallewegssystem gegen-über der Gesamtzhl an Operationen.

2.
Die Quote an Carcinomen der Papilla Vateri stieg an, die des Gallenblasencarcinoms blieb gleich.

3.
Altersmäßig lag das Maximum der Operationen am Gallenwegssystem im 6. Dezennium, das Maximum der Carcinome am Gallenwegssystem ein Jahrzehnt später. Vor dem 40. Lebensjahr wurde kein Carcinom am GWS gefunden.

4.
Wichtige Werte waren bei den einzelnen, nach ihrer Lokalisation differenzierbaren Carcinomgruppen am Gallenwegssystem unterschiedlich groß.

a)
Das Gallenblasencarcinom fand sich nur bei Frauen, das Papillencarcinom was dagegen bei Frauen nur 11/2 mal so oft wie bei Männern zu finden.

b)
Lebermetastasen traten häufiger auf, wenn das Carcinom lebernah lokalisiert war: Gallenblasencarcinome 15%, Carcinome an der Leberpforte 14%. Beim Choledochuscarcinom und Papillencarcinom waren Lebermetastasen nur in 0% bzw. 8% der Fälle zu finden.

c)
Das Adenocarcinom war die häufigste histologische Form der Carcinome am Gallenwegssystem. Dann folgte das solide Carcinom. Das galt beim Gallenblasencarcinom ausgeprägter (3,5fach) als bei den anderen Carcinomgruppen am Gallenwegssystem.

d)
Die größte Patientengruppe hatte beim Gallenblasencarcinom eine Beschwerdedauer von 7 bis 12 Monaten, bei den anderen Carcinomgruppen am Gallenwegssystem eine Beschwerdedauer von 2 bis 6 Monaten, aber auch längere Anamnesen wurden gefunden.

e)
Gallenkoliken traten beim Gallenblasen- und Leberpfortencarcinom häufiger auf als beim Choledochus- und Papillencarcinom.
Die Zunahme der Gallenwegscarcinome in den letzten Jahren ist in unserem Material fast ausschließlich durch eine Vermehrung der Carcinome an der Papilla Vateri bedingt, während die Zahl der Gallenblasencarcinome gleich geblieben ist. Die Papillencarcinome lassen sich aber häufig nur schwer von Tumoren des Pankreaskopfes trennen; außerdem unterliegen die Papillencarcinome auch nicht nur Einflüssen aus dem Bereich der Gallenwege. Deshalb müssen die Mitteilungen über eine Zunahme Carcinome am Gallenwegssystem mit Zurückhaltung betrachtet werden. Ohne Differenzierung des Tumorsitzes könnten sie zu falschen Schlüssen verleiten.
Es erscheint nicht sinnvoll, die Carcinome am Gallenwegssystem als Einheit zu betrachten, obwohl die Seltenheit der einzelnen Carcinomgruppen am Gallenwegssystem immer wieder dazu verführt, sie zusammenzufassen.
Es wird empfohlen, die nach der Lokalisation unterscheidbaren Carcinomgruppen des Gallenwegssystems auch bei geringer Fallzahl getrennt darzustellen, um eine Grundlage für eine aussagekräftige Sammelstatistik zu erhalten.
