Summary

The purpose of these studies was to examine the changes in sudomotor patterns in the skin of the human trunk produced by experimentally induced irritation and stresses, in the musculoskeletal tissues, as a step toward understanding the origins of the patterns found in apparently normal individuals.

Irritation of the musculoskeletal tissues was produced by the injection of hypertonic saline into paravertebral tissues. Postural stresses were produced by the insertion and removal of heel lifts and by the lateral inclination of the pelvis by the use of tilt-chairs.

Sudomotor patterns were revealed by recording electrical skin resistance on the dorsal skin of the thoracic and lumbar regions in relation to segmental level.

4.
New areas of low electrical resistance appeared when the saline injection produced referred pain; the areas were distributed in the reference zone, in dermatomes related to the injection site.

5.
Postural changes produced changes in patterns which included a) exaggeration of existing patterns and b) the appearance of new areas of low resistance (increased sweat gland secretion), according to the applied stress, the individual's vertebral adaption to the stress and the areas of discomfort.

We believe the findings support the following hypotheses:

a)
That the manifestations of altered sympathetic activity observed in these studies represent distortions of normally existing patterns of efferent activity.

b)
That the distortions begin, as responses to exaggerations of segmental or local afferent influences which ordinarily have only local adjustive influences on the patterns, and that these impulses may be visceral or somatic in origin.

c)
That although the aberrant areas of low resistance described in normal subjects, may reflect chronically altered or intensified patterns of afferent bombardment from foci in visceral or somatic tissues, other factors, such as adaptive or pathological changes in those tissues and altered central excitability, may eventually become involved.

Since, however, the affected areas may be limited in extent to single dermatomes, it appears that individual ganglia, gray rami communicantes ventral roots, spinal nerves or their branches may in some cases bedirectly, rather than reflexly, irritated.

Some of the functional implications of chronically altered activity in localized portions of the sympathetic outflow are identified.

9.
The relation of these findings to mechanisms involved in such clinical phenomena as reflex sympathetic dystrophy, myofascial triggers, referred pain, etc., is briefly examined.
Zusammenfassung

1.
In den vorliegenden Untersuchungen sollten die durch experimentelle Reizungen oder Stress im Skelettmuskel hervorgerufenen Änderungen der sudomotorischen Muster in der Haut des menschlichen Rumpfes untersucht werden, um dadurch Anhaltspunkte für das Verständnis des Ursprungs der bei anscheinend normalen Individuen auftretenden Muster zu erhalten.

Die Reizung der Skelettmuskelgewebe wurde durch Injektion einer hypertonischen Kochsalzlösung in das paravertebrale Gewebe erzeugt. Lagebedingter Stress wurde durch Anlegen und Entfernen von Fersenhebern sowie durch Seitwärtsneigen des Beckens mittels Kippstühle hervorgerufen.

Sudomotorische Muster wurden durch Registrierung des elektrischen hautwiderstandes in der Rückenhaut der thoracalen und lumbalen Regionen in bezug auf die Segmentalebene erhalten.

Neue Areale mit niedrigem elektrischem Widerstand ergaben sich, wenn die injizierte Salzlösung einen Bezugsschmerz hervorrief; die Areale waren in der Bezugszone in Dermatomen verteilt, die mit dem Injektionsort in Beziehung standen.

5.
Lageveränderungen bewirkten Veränderungen der Muster folgender Art: a) Übermä\ige Verstärkung bestehender Muster und b) Auftreten neuer Areale mit niedrigem Widerstand (vermehrte Schweißsekretion), entsprechend dem angewandten Stress, der individuellen vertebralen Anpassung auf den Stress und entsprechend den betroffenen Arealen.

Die Autoren glauben, daß die erhobenen Befunde folgende Hypothesen unterstützen:

a)
Die Manifestationen geänderter sympathischer Aktivität, die bei diesen Studien beobachtet wurden, stellen Verzerrungen normalerweise bestehender Muster efferenter Aktivität dar.

b)
Die Verzerrungen dieser Muster beginnen als Reaktionen auf eine gesteigerte Aktivität segmentaler oder lokaler afferenter Einflüsse, die ursprünglich nur lokale anpassende Wirkungen auf die Muster haben; die Impulse sind visceralen oder somatischen Ursprungs.

c)
Obwohl die aberranten Areale mit niedrigem Widerstand bei normalen Individuen beschrieben sind, könnten sie chronisch alterierte oder intensivierte Muster eines afferenten „Bombardements” von focalen Herden in visceralen oder somatischen Geweben widerspiegeln; auch andere Faktoren, wie adaptive und pathologische Veränderungen in solchen Geweben und eine geänderte zentrale Erregbarkeit, können eventuell enthalten sein.

Da jedoch die betroffenen Areale in ihrer Ausdehnung auf einzelne Dermatome beschränkt werden können, scheint es, daß individuelle Ganglien, Rami communicantes grisei, ventrale Wurzeln, Spinalnerven oder deren Zweige in einigen Fällen eher direkt als reflektorisch gereizt werden.

8.
Einige funktionelle Folgerungen chronisch veränderter Aktivität in lokalisierten Anteilen der sympathischen efferenten Bahnen werden gezogen.

9.
Die Beziehung dieser Befunde zu Mechanismen, die an solchen klinischen Phänomenen beteiligt sind, wie reflektorische sympathische Dystrophie, myofasciale „Triggers”, Bezugsschmerz usw., werden kurz untersucht.

Le but de ces études était l'examen des changements de dessins sudomoteurs dans la peau du tronc humain produits par une irritation expérimentalement induite et par des stresses dans les tissues musculosquelettiques, comme un pas vers la compréhension des origines des dessins trouvés dans des individus apparement normaux.

L'irritation des tissues musculosquelettiques a été produite par l'injection d'une solution saline hypertonique dans des tissus, paravertébaux. Des stresses de posture ont été produits par l'insertion et l'enlèvement de levées de talon et par l'inclination latérale du bassin moyennant des chaises basculantes.

Des dessins sudomoteurs ont été révélés par l'enregistrement de la résistance électrique de la peau sur la peau dorsale des régions thoraciques et lombaires relativement au niveau des segments.

4.
De nouvelle aires d'une basse résistance électrique ont apparu quant l'injection saline avait produit des douleurs de référence; les aires étaient distribuées dans la zone de référence, dans des dermatomes en relation avec l'endroit de l'injection.

Des changements de posture ont produit des changements de dessin qui comprenaient: a) l'exagération de dessins existants et b) l'apparence de nouvelles aires de basse résistance (sécrétion augmentée des glandes sudorifiques), selon le stress appliqué, l'adaption vertébrale de l'individu au stress et les aires de l'incommodité.

Nous sommes d'avis que les constatations sont en faveur des hypothèses suivantes:

a)
Les manifestations de l'activité sympathique observées dans ces études représentent des distorsions de dessins d'activité efférente normalement existants.

b)
Les distorsions commencent comme des réponses à des exagérations d'influences afférentes segmentaires ou locales qui, d'ordinaire, n'exercent que d'influences ajustives locales sur les dessins. Ces impulsions peuvent être d'origine viscérale ou somatique.

c)
Quoique les aires aberrantes de basse résistance décrites dans des personnes normales puissent refléter des dessins chroniquement altérés ou intensifiés de bombardement afférent partant de foyers dans des tissus viscéraux ou somatiques, d'autres facteurs, comme des changements adaptifs ou pathologiques dans ces tissus et une excitabilité centrale altérées peuvent éventuellement participer.

Comme, pourtant, les aires affectées peuvent être limitées en extension à des dermatomes séparés, il semble que des ganglions individuels, des rameaux communicants gris, des racines ventrales, des nerfs spinaux ou leurs branches, peuvent être irrités, en quelques cas, directement plutôt que réflexement.

8.
Quelques-unes des implications fonctionnelles de l'activité chroniquement altérées dans des parties localisées des voies sympathiques efférentes sont identifiées.

9.
Le rapport de ces constatations à des mécanismes participant dans de tels phénomènes cliniques, comme la dystrophie sympathique réflexe, les déclics myofasciaux, la douleur de référence etc., est brèvement examiné.
