Präoperative percutane transhepatische Gallenwegsdrainage (PTCD) beim malignen Verschlußikterus

Summary

Percutaneous transhepatic intubation and drainage of the biliary system were performed in 23 patients, 19 cases preoperatively because of malignant obstructive jaundice. Within 12 days the bilirubin serum level went to normal values, and the patients general condition and liver function showed marked improvement. By preoperative PTCD the mortality of palliative procedures in pancreatic carcinoma decreased. Complications occurred in five cases and were easy to manage. Clinical aspects such as indication, optimal period of drainage, and value of other techniques in bile drainage are discussed.

Zusammenfassung

Bei insgesamt 23 Patienten wurde eine percutane transhepatische Cholangiodrainage (PTCD) durchgeführt. In 19 Fällen präoperativ bei malignem Verschlußikterus mit Bilirubinwerten über 150 mg/l. Nach 12 tägigem Gallefluß von 50–2200 ml/die waren die Bilirubinspiegel deutlich abgesunken und der Allgemeinzustand der Patienten war wesentlich gebessert. Durch die PTCD präoperativ konnte die Letalität palliativ operierter Patienten mit Pankreascarcinom drastisch gesenkt werden. Leicht zu beherrschende Komplikationen traten 5mal auf. Klinische Gesichtspunkte wie die Indikation zur PTCD, die optimale Drainagedauer und der Wert anderer Techniken der Gallenwegsdrainage werden diskutiert.
