Akute nekrotisierende Einschlußkörperchen-Encephalitis

Isolierung von Herpes simplex-Virus aus dem Gehirn

Zusammenfassung

Kasuistischer Bericht über eine virologisch-morphologisch verifizierte Herpes simplex-Encephalitis des Erwachsenen mit tödlichem Ausgang: 18 1/2 jährige Frau, die nach grippalen Prodromen mit Fieber, Bewußtseinstrübung, fokalen Krampfanfällen und Halbseitenzeichen erkrankte und nach 9 Tagen im cerebralen Koma verstarb. Typischer EEG-Befund. Autoptisch pseudomembranöse Tracheobronchitis, Bronchopneumonie und akuter Milztumor. Im Gehirn das Bild einer akuten nekrotisierenden Encephalitis mit typischer Betonung der entzündlich-nekrotisierenden Läsionen in den basalen Hirnabschnitten sowie typischem Kerneinschlußkörperbefund in neuroektodermalen und mesodermalen Zellen. Die postmortale Isolierung eines als Herpesvirus hominis identifizierbaren Erregers aus dem Hirngewebe ergibt einen weiteren Beweis für die ätiologische Identität der akuten nekrotisierenden Einschlußkörper-Encephalitis und der Herpesencephalitis.

Summary

Report of a virologically and morphologically proven case of herpes simplex encephalitis in adult with lethal course: 18 1/2 year old woman after grippal prodroma developed fever, confusion, focal convulsions and slight hemiparesis. Death occurred in deep cerebral coma 9 days after the onset of the illness. Typical EEG-findings. The autopsy findings were: pseudomembranous tracheobronchitis, bronchopneumonia and acute tumor of the spleen. The brain lesions were those of an acute necrotizing encephalitis with topographic prevalence of the inflammatory necroses to the basal regions. Type A intranuclear inclusion bodies were found in neuroectodermal and mesodermal cells. The postmortal isolation of an agent identified as herpes virus hominis from brain tissue gives new evidence for the etiological identity of acute necrotizing encephalitis and herpes simplex encephalitis.
