Adenomatoid tumors (AdTu) are rare and usually benign tumors of the male and female genital tract. So far less than 100 cases have been reported in females compared to about 200–300 cases in men.

Three further cases of AdTu of the female genital region (Fallopian tube 2x, uterus 1x) are reported. In addition, pertinent literature is reviewed, and the following topics are discussed:
Frequency, localization, age and sex distribution, clinical symptomatology. pathology (macroscopy, microscopy incl. cytology), prognosis, and histogenesis,

Usually AdTu in women are observed in a later age than in men. This is probably due to the fact that AdTu in men are easily palpable in most cases (because of their localization in the scrotum) while AdTu in women are accidentally diagnosed during the operation of uterus myomatosus or microscopic examination of the operation specimen. In men, AdTu are localized most often within the epididymis, in women within the wall of the Fallopian tube.

Macroscopically, the AdTu appears as a round or ovoid tumor of grey or grey-white color. In women, the tumor is often misinterpreted as a leiomyoma of the uterus or Fallopian tube. Microscopically, three types are distingushed: A solid, macro- and microglandular (-follicular) type. The tumor is characterized by glandlike spaces (therefore “adenomatoid tumor”), lined by flat, cuboid or cylindric cells. The plasma of these cells tends to be vacuolized, the spaces are probably caused by confluence of these vacuoles. The interstitial tissue contains the same cells, collagenous connective tissue, elastic and reticulin fibers and—not in all cases—smooth muscle fibers.

Usually, AdTu are benign. Only few cases of pretentedly malignant AdTu have been reported, but the classification of these tumors remains doubtful.

As a rule, AdTu do not produce clinical symptoms, especially in women. In men, they may cause slight pain or a hydrocele testis. In most cases, the physician is consulted because of a palpable enlargement of the epididymis or a palpable node within the testis.

Several hypotheses concerning the histogenesis of AdTu are reviewed. Among these, the endothelial and mesothelial hypothesis have been discharged. The mesonephrogenous development (from parts of Wolff's or Gartner's duct) is discussed. Probably AdTu arise from remnants of the Müllerian duct, but the question whether epithelium or mesenchyma are the matrix of the tumor, has not yet been settled.

Adenomatoidtumoren (AdTu) sind seltene und in der weit überwiegenden Mehrzahl gutartige Geschwülste des männlichen und weiblichen Genitaltraktes. Bisher wurden weniger als 100 fälle bei der Frau beschrieben. Die Zahl der bei Männern beobachteten AdTu dürfte wenigstens zwei- bis dreifach höher liegen.

Die vorliegende Arbeit stützt sich a) auf drei Beobachtungen von AdTu des weiblichen Geschlechts — zweimal in der Tube, einmal im Uterus — sowie b) auf eine eingehende Literaturübersicht, in der folgende Eigenschaften der AdTu besprochen werden: Häufigkeit, Lokalisation, Alters- und Geschlechtsverteilung, Klinik, Pathologische Anatomie (Makroskopie, Mikroskopie einschließlich Cytologie), Prognose und Histogenese.

Die Tatsache, daß AdTu bei der Frau im Mittel zu einem späteren Zeitpunkt des Lebens beobachtet werden als beim Manne, wird auf die Besonderheiten der Lokalisation zurückgeführt. Beim Manne sind sie wegen der Lokalisation im Scrotum (Nebenhoden, Hoden, Tunica albuginea, Tunica vaginalis) eher palpabel, bei der Frau werden sie meist als Zufallsbefund während einer Myomoperation bzw. durch die histologische Untersuchung eines Uterus myomatosus erkannt. Unter den verschiedenen Lokalisationen führt beim Manne mit großem Abstand der Nebenhoden (etwa 85%), bei der Frau mit geringerem Abstand die Tube (etwa 58%). Selten tritt der AdTu primär multipel auf.

Makroskopisch imponiert der Tumor als eine meist rundlich-ovale und gut begrenzte Geschwulst von gewöhnlich grauweißer Farbe, die bei der Frau häufig mit einem Leiomyom des Uterus oder der Tube verwechselt wird. Mikroskopisch werden drei Typen unterschieden: Ein solider, mikro- und makrofollikulärer Typ. Charakteristisch ist die Bildung drüsenähnlicher Hohlräume (daher „AdenomatoidTumor“), die von flachen bis zylindrischen Zellen ausgekleidet werden. Die Zellen neigen zur Vacuolisierung, die Hohlräume werden als Folge der Konfluenz der Vacuolen gedeutet. Das Zwischengewebe enthält die gleichen Zellen sowie kollagenes Bindegewebe, elastische und Reticulinfasern, schließlich in wechselnder Menge und nicht in allen Tumoren glatte Muskulatur.

Die AdTu sind in aller Regel gutartige Geschwülste. Es gibt nur wenige Fälle angeblich maligner AdTu, bei denen z.T. Zweifel an der Klassifikation des Tumors bestehen.

Der AdTu erzeugt in der Regel keine klinischen Symptome. Dies gilt zumindest für die bei Frauen auftretenden Tumoren. Beim Manne kann er zu — meist geringfügigen — Schmerzsymptomen und zur Bildung einer Hydrocele testis führen. In den meisten Fällen ist jedoch die tastbare Vergrößerung des Nebenhodens bzw. die Knotenbildung im Hoden Anlaß für das Aufsuchen des Arztes.

Von den verschiedenen Hypothesen zur Histogenese der AdTu sind die Hypothese der endothelialen und weiterhin auch der mesothelialen Entstehung heute verlassen. Die mesonephrogene Genese (aus Teilen des Wolffschen bzw. Gartnerschen Ganges) wird diskutiert. Am wahrscheinlichsten ist die Entstehung der AdTu aus Teilen des Müllerschen Ganges, wobei umstritten ist, ob das Epithel oder das Mesenchym Ausgangspunkt der Tumorbildung ist.
