Summary
In 9 dog hearts (coronary arteries cannulated, aortic blood pressure allowed to change) an increase in coronary perfusion pressure raised the peak pressure in the left ventricle and the maximum ofdp/dt, while the left ventricular enddiastolic pressure decreased.
An enhancement of coronary perfusion pressure in 6 dog hearts (coronary arteries cannulated, aortic blood pressure kept constant) increased the peak pressure in the left ventricle and the maximum ofdp/dt; the left ventricular enddiastolic filling pressure decreased.
This coronary perfusion pressure-induced increase in heart performance can also be shown by ventricular function curves. These are relatively flat at low coronary pressures. Therefore, even large changes in left ventricular enddiastolic pressure bring about only a small increase in stroke work. Heart failure could not be observed at low coronary perfusion pressures. An increase in coronary perfusion pressure shifted the ventricular function curves to the left, so that even a small change in enddiastolic filling pressure had a great effect on strokework.
The “homeometric autoregulation” of the heart can be explained to a certain part by the coronary perfusion pressure.
There is no explanation of the effect of the coronary perfusion pressure on heart performance. Based on our findings it must be concluded that an increased coronary perfusion pressure distends the coronary arteries (“garden-hose-effect”), which increases myocardial fiber tension and heart performance.
Zusammenfassung
An 9 Hundeherzen mit kanülierten coronararterien führte eine Steigerung des coronaren Perfusionsdruckes von 85±8 mmHg auf 131±9 mmHg (N=25) zu einer Zunahme des Druckes im linken Ventrikel um 15±3 mmHg (N=25) und des Differentialquotientendp/dt max um 529±78 mmHg/sec (N=25). Gleichzeitig sank der enddiastolische Füllungsdruck im linken Ventrikel um −2,0±0,5 mmHg ab (N=25). Eine Steigerung des coronaren Perfusionsdruckes von 61±11 auf 101±11 mmHg (N=8) an Hundeherzen mit kanülierten Coronararterien und einem konstanten Aortendruck hatte eine Zunahme des Druckes im linken Ventrikel um 9±1 mmHg (N=8) und desdp/dt max um 531±120 mmHg/sec (N=8) zur Folge. Der enddiastolische Füllungsdruck sank gleichzeitig um −4,9±1,2 mmHg ab (N=8). Dieser leistungssteigernde Effekt des coronaren Perfusionsdruckes kann ferner anhand ventrikulärer Funktionskurven des Herzens gezeigt werden. Unabhängig davon, ob sich der mittlere Aortendruck ändert oder konstant gehalten wurde, zeigen die Funktionskurven bei niedrigen Perfusionsdrucken einen flachen Verlauf: Große Änderungen des enddiastolischen Füllungsdruckes befähigen den linken Ventrikel nur zu einer geringen Steigerung der Schlagarbeit. Ein Absinken der Schlagarbeit trotz steigenden Füllungsdruckes als Zeichen einer Insuffizienz des Herzens konnte auch bei den niedrigen Perfusionsdrucken nicht beobachtet werden.
Eine Steigerung des coronaren Perfusionsdruckes verschiebt die Funktionskurven im Diagramm nach links und oben; die Kurven verlaufen wesentlich steiler, so daß geringe Zunahmen des Füllungsdruckes zu einem starken Anstieg der Schlagarbeit führen.
Die “homeometrische Autoregulation” des Herzens kann zu einem großen Teil über den coronaren Perfusionsdruck erklärt werden. Danach befähigt der Perfusionsdruck das Herz, die infolge einer arteriellen Widerstandserhöhung vermehrte Herzarbeit zu halten, obgleich der enddiastolische Füllungsdruck absinkt.
Eine gültige Erklärung, warum der coronare Perfusionsdruck leistungssteigernd wirkt, läßt sich zur Zeit noch nicht geben. Aufgrund unserer Untersuchungen muß vermutet werden, daß der gesteigerte Perfusionsdruck zu einer Streckung der Coronargefäße führt (“Gartenschlaucheffekt”), die ihrerseits eine vermehrte Faserspannung und damit eine vergrößerte Kontraktionskraft zur Folge hat.
