Summary
Morphologic findings in the Prader-Labhart-Willi syndrome are described, based on two autopsy cases and a testicular biopsy of a third case. The known clinical manifestations are imbecility, obesity, growth retardation, hypogonadism and hypogenitalism, acromicria, dolichocephalus, an adult-type of diabetes mellitus and a history of myatonia.
There was testicular atrophy in varying degrees, with hyperplasia of Leydig cells, some of which were immature. The morphological findings correlated well with the degree of gonadotropic activity determined biologically for the individual cases. An increase in the number of mast cells was observed in many organs, especially in the lipoid rich interstitial tissue of the testes. Morphometric analysis of the pituitary was used to exclude a severe hypoactivity of this organ.
Vascular changes did not absolutely parallel the degree of diabetes, but were exceptionally severe in the case of the 28 year old patient with manifest diabetes. The PAS positive homogeneous lesions (Funk) of the capillaries were found in two cases. The persistence of the thymus was until now unknown, as was the lack of glycogen in the liver and myocardium observed in one case. This finding must be confirmed. Bone maturation is not disturbed.
The possibility of a falsely directed insulin effect in this syndrome is discussed. Such a disturbance might explain the peculiar sequence of myatonia in the early phase, the obesity and growth retardation in childhood, followed by the diabetes mellitus in early adulthood. The presumptive basic disturbance is postulated as a deranged fat metabolism and consequently inadequate insulin action.

Morphologische Analyse des Prader-Labhart-Willi-Syndroms an Hand von zwei Autopsien und der Hodenbiopsie eines 3. Falles. Neben den bekannten Befunden — Imbezillität, Adipositas, Kleinwuchs, Hypogonadismus und Hypogenitalismus, Myatonieanamnese, Akromikrie und Dolichocephalie, Diabetes vom Alterstypus — ist das wechselnde Ausmaß der Hodenatrophie mit teilweise unreifzelliger Leydigzell-Hyperplasie hervorzuheben. Diese stimmt mit der gonadotropen Aktivität in den einzelnen Fällen recht gut überein. Daneben findet sich in zahlreichen Organen eine Mastzellenvermehrung, besonders auch im lipoidreichen Hodeninterstitium. Die morphometrische Hypophysenanalyse läßt deren Hypaktivität ausschließen. Ein auffälliger Hirnbefund in Fall 1 und 2 bedarf der Bestätigung durch weitere Fälle.
Die Gefäßveränderungen sind außergewöhnlich schwer im Falle des 28jährigen Patienten mit manifestem Diabetes. Unbekannt waren bisher die Thymuspersistenz und die noch zu bestätigende Glykogenlosigkeit in Leber und Myokard des einen daraufhin untersuchten Falles. Eine Störung der Knochenreifung besteht nicht. Im komplikationsreichen Fall 1 mit Diabetes bestand eine asymptomatische Osteoporose mit Kyphoskoliose der Wirbelsäule.
Die mögliche Bedeutung einer fehl geleiteten Insulinwirkung für die eigenartige Sequenz von Myatonie im Frühstadium, Adipositas und Kleinwuchs im Kleinkindesalter und Diabetes mellitus im 2. Lebensjahrzehnt wird diskutiert und die vermutliche Basisstörung in einer Noxe des Fettstoffwechsels und der Insulinwirkung gesehen.
