Grundlagen der plastisch-chirurgischen Versorgung von Defekten im Schädel- und Gesichtsbereich

Basic techniques of plastic surgical repair in defects of the skull and face

Summary

Ommitting historical datas, ancient theories and old-fashioned surgical techniques, the author gives a summary of current plastic surgery in the head and neck.

After a review of aesthetic characteristics of the face, basic principles of surgical skin closure and different forms of local and distant flaps are discussed. Emphasis is placed on tissue transplantation. The biological problems in transplantation of skin, subcutaneous and supporting tissues, using autografts, allografts and xenografts are mentioned in addition to several methods of conservation.

The clinical importance of various autogenous supports used for transplantation to the skull and face is stressed.

Concerning foreign implants, the author discusses the problem of the so-called foreign-body sarcoma and the foreign-body reaction to synthetic and metallic implants. Indications for using synthetic implants are mentioned. Microsurgical techniques seem to be important for further development of facial plastic surgery.

Zusammenfassung

Im vorliegenden Referat versucht Verf. unter Verzicht auf die Wiedergabe historischer Daten und inzwischen als überholt geltender Hypothesen und Operationstechniken den gegenwärtigen Standort der plastischen Chirurgie im Gesichts-Schädel-Bereich darzulegen. Nach der Besprechung ästhetisch relevanter Besonderheiten der Gesichtsregion und den Grundprinzipien der chirurgischen Hautnaht werden die verschiedenen Formen der Nah- und Fernlappenplastik abgehandelt. Breiten Raum nimmt die Besprechung der freien Gewebeverpflanzungen ein. Neben der Erörterung der biologischen Vorgänge nach autologer, homologer und heterologer Verpflanzung von Haut-, Subkutan- und Stützgewebe und Schilderung der Konservierungsmethoden wird u. a. auch zur Frage der Wertigkeit der verschiedenen körpereigenen Stützmaterialien im Schädel- und Gesichtsbereich Stellung genommen. In dem Kapitel über die Implantation unbelebter Werkstoffe geht Verf. zunächst etwas ausführlicher auf das Problem des sog. Fremdkörpersarkoms und auf die nach der Einlagerung von Kunststoffen und Metallen zu erwartenden Fremdkörperreaktionen ein, um dann einzelne Indikationsbereiche für die Verwendung von Werkstoffen aufzuzeigen. Hinweise auf die Bedeutung der mikrochirurgischen Gefäßanastomosierung für die Weiterentwicklung der plastischen Gesichtschirurgie beschließen das Referat.
