Summary
Based on our partial material of 767 total prosthetic replacement arthroplasties some aspects of the operative technique are discussed which may arise under unfavourable local conditions. Specific mention is given to problems contributable to marked coxarthrosis, trauma and following previous hip surgery. Extremely flat dysplastic acetabula cannot be provided for satisfactorily with the presently available types of artificial sockets. Recent traumatic acetabular fragmentations should be subjected to alloarthroplasty only after consolidation. Painful partial ankylosis may be individually furnished with arthroplasty. However, we cannot yet decide on immediate prosthetic reimplantation in case of deep infection.
Zusammenfassung
Unter Auswertung eines Teils unseres Krankengutes von 767 Hüftarthroplastiken mit Totalprothesen werden operationstechnische Fragen besprochen, die sich bei ungünstigen lokalen Verhältnissen ergeben können. Im einzelnen wird auf Schwierigkeiten eingegangen, die im Rahmen starker Coxarthrosen, traumatischer Schäden und im Anschluß an vorausgegangene Hüftoperationen zu erwarten sind. Bei extrem flachen Dysplasiepfannen lassen die derzeit zur Verfügung stehenden Kunstpfannen eine befriedigende Versorgung nicht zu. Frische Pfannenzertrümmerungen sind erst nach knöcherner Abheilung für die Totalalloplastik geeignet. Die arthroplastische Versorgung schmerzhaft wackelsteifer Hüften ist im Einzelfall indiziert und technisch durchführbar. Zur sofortigen Reimplantation bei infizierten Totalendoprothesen können wir uns derzeit noch nicht entschließen.
