Die Synovektomie der Hüfte

The synovectomy of the hip

Summary

The results of 25 synovectomies of the hip joint are reported. The diagnosis was in the most cases a monoarthritis of the hip. The assessment bases on early results so that the progression of destruction of the joint could not be followed up. Pain relief was as a rule very good. The technic of operation (partial synovectomy, anterior-laterally approach, avoiding luxation of the head) is discussed. None necrosis of the head was observed. Following this review emphasis is laid on early diagnosis and early therapy. The late synovectomy is also recommanded by younger patients, so that the total hip prosthesis can be post-posed.

Zusammenfassung

Die Frühergebnisse von 25 Synovektomien wurden analysiert. Das Krankengut bestand hauptsächlich aus sog. Monoarthritis coxae. Wenn auch die kurze Beobachtungszeit keine Aussage über die Auswirkung des Eingriffs über die Progredienz der Arthritis und über die fortschreitende Destruktion des Gelenkes ermöglicht, war die Beseitigung der Schmerzen eindrucksvoll. Unter Berücksichtigung von bestimmten Operationsbedingungen (partielle Synovektomie, ventro-lateraler Zugang, keine Luxation des Hüftkopfes) erwies sich die Gefahr der Hüftkopfnekrose gleich Null. Die ansonsten im Bereich anderer Gelenke geltenden Prinzipien der Frühdiagnose und der Frühsynovektomie scheinen im Bereich der Hüfte durchaus anwendbar. Aber auch die Spätsynovektomie, die Schmerzfreiheit gewährleistet, ist bei jüngeren Patienten berechtigt, um den Zeitpunkt der Totalendoprothesenversorgung so weit als möglich hinauszuschieben.
