Summary
28 children with birth palsy and 16 children with strictly unilateral poliomyelitis treated at the Orthopedic Clinic of the University of Heidelberg, were examined in a clinical and radiographical study 2 to 14 years after the onset of palsy. As a measure of bone size the superficial index was calculated for each bone of carpus and epiphyseal nucleus and compared with the standard values by Schmid and Moll and Schulte-Brinkmann. In the long and short bones of the upper limbs inequality in length and width was measured.
Taking into consideration the highest respectively lowest level of norm we found that in the non paralised limbs in the average the mass of the carpal bone was more, that was 19% in cases with plexus paralysis and 2.4% in cases with poliomyelitis, while in paralised limbs the mass was less, in fact 2.7% respektively 0.5%. When the measures of bones in paralised and non paralised bones were compared, it became clear that the growth in width was more reduced than in length. By our findings we came to the conclusion that there is a correlation between the severity, expansion, and localization of the plasy and the degree of growth retardation. The duration of palsy plays no important role for the growth deficiency. It seems that the birth palsy follows a non progressive type of course. Only in few cases of non paralised hands and more often in paralised hands we observed disturbance in sequence, pseudoepiphysis and so called cushion metaphysis.
Zusammenfassung
28 Kinder mit Geburtslähmungen und 16 Kinder mit streng einseitigen Poliolähmungen, die zwischen 1954 und 1968 in der Orthopädischen Universitätsklinik und Poliklinik Heidelberg stationär behandelt worden waren, wurden klinisch und röntgenologisch 2–14 Jahre nach Lähmungseintritt nachuntersucht. Für jeden Handwurzelknochen und Epiphysenkern wurde als Maß der Knochengröße der Flächenindex berechnet und mit der Gegenseite sowie mit Normwerten von Schmid u. Moll und Schulte-Brinkmann verglichen. An den langen und kurzen Röhrenknochen der oberen Extremität wurden Längen- und Breitendifferenzen bestimmt. Im Mittel waren die Handwurzelknochen auf der gesunden Seite bei Plexuslähmungen um 10% und um 2,4% bei Poliolähmungen größer, während sie auf der gelähmten Seite um 2,7 bzw. 0.5% kleiner als die oberen bzw. unteren Grenzwerte des Normbereiches waren. Der Vergleich der Knochenmaße an gesunder und kranker Seite ergab, daß das Wachstum in der Breite mehr gehemmt ist als in der Länge und daß eine gewisse Beziehung zwischen Schwere, Ausdehnung und Lokalisation der Lähmung einerseits und der Wachstumsverzögerung andererseits besteht. Die Dauer der bestehenden Lähmung spielt für das Wachstumsdefizit keine wesentliche Rolle; die Geburtslähmung folgt anscheinend einem sogenannten nichtprogressiven Verlaufstyp. In wenigen Fällen fanden wir an der gesunden und häufiger an der gelähmten Hand Reihenfolgestörungen, Pseudoepiphysen und Polstermetaphysen.
