Vergleich von Provokationstests und Tonographie in der Glaukomdiagnostik

Summary

The results of the provocative tests and of the tonography on eyes with incipient simple chronic glaucoma were evaluated according to the improved criteria (Makabe, 1970, Albrecht v. Graefes Arch. klin. exp. Ophthal.) and compared with each other. Between the results of the tonography and of the waterdrinking test, there was a significant positive correlation, which could not be found between the results of the tonography and of the priscol test. Owing to this difference of the reaction, the incidence of the pathologic response to the priscol test (57%) could be more improved than that of the waterdrinking test (69%), as we took into consideration the results of the tonography. The total incidence reached in both cases to 84%. The results of the tonography showed to those of the mydriasis and of the darkroom test a certain similarity in the diagnostic significance, and to those of the caffeine test no similarity.

The intraocular pressure elevation at the waterdrinking test was more influenced by the heigt of the intraocular pressure than by the level of the resistance to the outflow of the aqueous humour. On the contrary the intraocular pressure elevation at the priscol test was more dependent on the level of the outflow resistance than on that of the intraocular pressure. The caffeine test acted similar to the priscol test. Between them and the waterdrinking test came the mydriasis and the darkroom test and formed a third group.

Zusammenfassung

Die nach den verbesserten Verfahren (Makabe, 1970, Albrecht v. Graefes Arch. klin. exp. Ophthal.) ausgewerteten Ergebnisse von Tonographie und Wassertrinktest an Augen mit beginnendem Glaucoma chronicum simplex standen in einer signifikanten positiven Beziehung, wie sie bei den Ergebnissen von Tonographie und Priscoltest nicht nachgewiesen werden konnte. Infolge dieses Reaktionsunterschiedes kann die diagnostische Ergiebigkeit deutlicher erhöht werden als die des Wassertrinktests, wenn man dazu die Ergebnisse der Tonographie berücksichtigt. Eine gewisse Ähnlichkeit in der Bedeutung für die Glaukomdiagnostik wurde auch zwischen der Tonographie und dem Mydriasis-oder Dunkelzimmertest beobachtet, die zwischen der Tonographie und dem Coffeintest nicht nachgewiesen werden konnte.

Der Druckanstieg im Wassertrinktest wurde mehr von der Höhe des intraocularen Druckes beeinflußt als von der Größe des Kammerwasserabflußwiderstandes. Hingegen war der Druckanstieg im Priscoltest mehr vom Abflußwiderstand abhängig als vom intraocularen Druck. Der Coffeintest verhielt sich wie der Priscoltest. Zwischen ihnen und dem Wassertrinktest ordneten sich Mydriasis- und Dunkelzimmertest ein und bildeten eine 3. Gruppe.
