Abstract
The preoperative shape of an eye affected by keratoconus can be evaluated by means of Placido photography of the cornea and the “Placido axis” of the main astigmatic error determined (Figs. 1, 2).
In 64% of 105 cases it was shown that the postoperative astigmatism correlated well with the preoperative Placido axis (Table 5). A perfectly circular graft if transplanted, will therefore again adopt the preoperative shape of the cornea.
From the facts above, a slightly oval graft sutured perpendicularly to the preoperative main Placido axis would probably compensate for this astigmatism but this has not been carried out experimentally as yet.
Zusammenfassung
Mit Hilfe der Placidofotografie wird die präoperative Form des Auges festgehalten und die sogenannte „Placido-Achse” des Keratokonus bestimmt.
In 64% von 105 Fällen konnte nachgewiesen werden, daß die postoperative Astigmatismus-Achse mit der präoperativen relativen Placidoachse übereinstimmt. Mit anderen Worten: wenn ein rundes Transplantat eingesetzt wird, bekommt das Auge postoperativ wieder seine präoperative Form. Mit einer experimentellen Anordnung konnte gezeigt werden, daß sich bei künstlich erzeugtem Astigmatismus die runde Trepanationsöffnung oval verzieht, und zwar um 90 ° zur präoperativen relativen Placidoachse.
Auf Grund dieser Feststellungen wird versucht, die Spenderhornhaut oval zu trepanieren und in einem Winkel von 90 ° zur präoperativen Placidoachse in die Trepanationsöffnung einzunähen.
