Summary
The case is reported of a demyelinating process in a 39 year-old woman. The disease lasted for 21/2 years and had lead to multiple neurological symptoms and an organic mental syndrome. Neuropathological examination revealed multiple, patchy, confluent, poorly demarcated lesions of demyelination within both cerebral hemispheres. Brain stem and spinal cord were not affected. No essential myelin breakdown products could be found within the demyelinated areas, and the axons were relatively preserved. There was a reduction of oligodendroglia. No pathological cell-elements could be found except for few intranuclear structures suggestive of inclusion-bodies. The astrocytes showed only mild proliferation with but few large protoplasmic cells and without proliferation of glial fibres.
The authors consider the main findings of this case to be consistent with “multifocal progressive leukoencephalopathy”. The less typical features could be explained by the unusually long duration of the disease.
Zusammenfassung
Bericht über eine schwierig einzuordnende Entmarkungskrankheit bei einer 39jährigen Frau, die im Verlauf von 21/2 Jahren zu multiplen neurologischen Ausfällen und einem organischen Psycho-Syndrom führte. Die neuropathologische Untersuchung ergab multiple unscharfe, fleckförmig-konfluierende subtotale Entmarkungen in beiden Großhirnhemisphären. Hirnstamm und Rückenmark waren nicht betroffen. Es fanden sich keine nennenswerten Abbauvorgänge; die Achsenzylinder waren im Verhältnis zur Schwere der Entmarkung relativ verschont. Die Oligodendroglia war in ihrem Bestand reduziert; pathologische Zellformen waren kaum vorhanden. Nur ganz vereinzelt konnten auf intranucleäre Einschlußkörperchen verdächtige Gebilde gefunden werden. Die Astroglia wies lediglich eine leichte Proliferation mit wenigen großen protoplasmatischen Zellelementen ohne Faserbildung auf.
Die Autoren sind der Meinung, daß die mitgeteilte Beobachtung weitgehend dem Bild der „multifokalen progressiven Leukoencephalopathie“ entspricht. Die abweichenden Befunde könnten durch die für diese Krankheit ungewöhnlich lange Verlaufsdauer dieses Falles erklärt werden.
