Hemoperfusion with XAD-4 resin in the treatment of a severe parathion intoxication

Abstract

After ingestion of 200–300 ml E 605-forte (i.e., 100–150 g parathion) a maximal concentration of 10.2 mg/l could be measured in the plasma of a male patient. An extracorporeal hemoperfusion was carried out using XAD-4 resin. The clearance valued 83 ml/min at a blood flow of 100 ml/min, which is about 40% higher compared to perfusion on activated charcoal. The described method is possibly an improved technique which could raise the rate of survival following parathion intoxication. In our study, however, the fatal end could not be prevented due to the intake of a very high dose of the poison.

Zusammenfassung

Bei einem Patienten, der eine extrem hohe Menge E 605 forte® getrunken hatte (etwa 200–300 ml Lösung entsprechend 100–150 g Parathion), betrug die maximale Plasmakonzentration des Wirkstoffes 10,2 μg/ml. Es wurde eine extrakorporale Hämoperfusion über XAD-4-Harz durchgeführt, die mit einer Clearance von 83 ml/min bei einem Blutfluß von 100 ml/min eine um etwa 40% höhere Eliminationsrate ergab als die bisher verwendete Aktivkohleperfusion. Die Überlebenschance bei Parathion-Vergiftungen könnte durch die neue Methode verbessert werden, im vorliegenden Fall war jedoch der letale Ausgang infolge der äußerst hohen Dosis des Giftes nicht zu verhindern.
