Zusammenfassung
Durch die demographische Entwicklung nimmt der Anteil der älteren Bevölkerung in den nächsten Jahrzehnten zu. Damit steigt auch die Prävalenz der Harninkontinenz. Diese wird oft nicht erkannt, da die Betroffenen ihre Beschwerden sowohl gegenüber Angehörigen als auch gegenüber Ärzten verschweigen und auch Ärzte nicht gezielt nach Symptomen der Harninkontinenz fragen. Aber auch wenn die Diagnose gestellt wurde, bleibt die Inkontinenzversorgung oft unzureichend. Eine sorgfältige Anamnese bildet die Grundlage für alle diagnostischen und therapeutischen Schritte. Im Vordergrund stehen konservative Untersuchungen und Behandlungen. Nur in wenigen Fällen ist bei älteren Patienten eine invasive Diagnostik indiziert. Da Ältere häufig mehrere Medikamente einnehmen, müssen mögliche Interaktionen und Nebenwirkungen besonders beachtet werden. Vor allem kognitive Einschränkungen durch die Therapie sollten vermieden werden. Die Inkontinenzchirurgie wird bei Älteren seltener durchgeführt, kann aber im Einzelfall sinnvoll sein.
