A rare cause of upper gastrointestinal hemorrhage

Summary

Background: The cause of acute gastrointestinal hemorrhage most frequently is located in the upper gastrointestinal tract. Previous surgical procedures such as intestinal anastomoses may be predisposing bleeding sites.

Methods: We report on a patient who had received gastroenteroanastomosis for duodenal atresia as a newborn. 35 years later she presented with recurrent gastrointestinal bleeding from an anastomotic ulceration.

Results: Endoscopic treatment was initially successful. Because of the predisposing anatomical situation of a gastrojejunostomy without Braun anastomosis surgical correction was recommend and performed 10 days later. 2 years post operation, the patient is now completely free of symptoms.

Conclusions: Even after long periods of time (>30 years), life-threatening upper gastrointestinal hemorrhage may occur after non-optimal neonatal surgery.

Zusammenfassung

Grundlagen: Die Ursache einer akuten gastrointestinalen Blutung findet sich meist im oberen Gastrointestinaltrakt. Intestinale Anastomosen können eine Prädilektionsstelle für Blutungen darstellen.

Methodik: Wir berichten über eine Patientin, die als Neugeborenes wegen einer Duodenalatresie eine Gastroenteroanastomose erhielt. Im Alter von 35 Jahren erlitt sie eine Rezidivblutung aus einem Anastomosenulkus.

Ergebnisse: Die Blutung konnte primär endoskopisch beherrscht werden. Aufgrund der ungünstigen anatomischen Situation einer doppelläufigen Gastrojejunostomie ohne Braunsche Anastomose wurde der Patientin eine chirurgische Korrekturoperation empfohlen und im Intervall durchgeführt.

Schlußfolgerungen: Auch nach langer Latenzzeit (> 30 Jahre) können lebensbedrohliche Anastomosenblutungen nach nicht optimaler chirurgischer Versorgung kongenitaler Defekte auftreten.
