Viral Retinitis-Related Retinal Detachment

Abstract

One of the most sight-threatening complications of necrotizing retinitis is the development of retinal detachment. Retinal detachment is unfortunately, a frequent complication that results from retinal thinning, atrophy, and necrosis, resulting in the formation of multiple holes and breaks. Common conditions associated with development of retinal detachment include acute retinal necrosis (which has a very high incidence of retinal detachment; >50%), and cytomegalovirus retinitis (which may or may not be associated with infection with human immunodeficiency virus). Management of viral retinitis-related retinal detachment poses a surgical challenge due to high rates of anatomical failure and development of re-detachment, as well as poor postoperative visual gains. Pars plana vitrectomy with or without additional scleral buckling and silicone oil tamponade is the most commonly used surgical technique for management. Most series in the literature indicate a guarded visual prognosis in eyes with viral retinitis-related retinal detachment.
