Sporadischer primärer Hyperparathyreoidismus

Zusammenfassung

Der primäre Hyperparathyreoidismus (pHPT) beschreibt eine Überfunktion der Nebenschilddrüsen, die in 85–90 % durch ein solitäres Nebenschilddrüsenadenom und seltener durch eine Mehrdrüsenerkrankung verursacht wird. Die typischen Symptome eines pHPT sind vermehrte Müdigkeit, Konzentrationsstörungen, Stimmungsschwankungen, Depressionen, Nephrolithiasis, Osteoporose und Hypertonie. Eine Heilung des pHPT kann nur durch eine Operation erfolgen. Um einen fokussierten Eingriff zu ermöglichen, erfolgt präoperativ eine Lokalisationsdiagnostik mit möglichst zwei kongruenten Verfahren (z. B. Sonografie und Mibi-Szintigrafie), alternativ kann eine bilaterale zervikale Exploration erfolgen. Die Heilungsraten liegen bei über 96 % und korrelieren in multizentrischen Studien mit der Erfahrung des Chirurgen. Postoperativ werden neben einer Remineralisierung des Knochens und Senkung des Risikos für eine weitere Nephrolithiasis, auch eine Verbesserung der gesundheitsbezogenen Lebensqualität und Rückgang der Depressivität nachgewiesen.
