Indikation, Technik und Ergebnisse der Sleeve Gastrektomie

Zusammenfassung

Die eigenständige Schlauchmagenoperation (LSG: laparoscopic sleeve gastrectomy) wurde zu der häufigsten Operationsart in der Behandlung der krankhaften Adipositas. LSG ist eine sichere und effektive primäre Adipositasoperation mit einem langanhaltenden Gewichtsverlust. Das einfache Konzept der Operation, bei der keine Darmanteile verändert werden müssen, macht die LSG zu der am häufigsten durchgeführten Operation weltweit. Nichtsdestoweniger sind feine Details der Operation entscheidend, um Komplikationen zu vermeiden. Die Kernpunkte einer sicheren LSG bestehen in einer Kalibration einer bis mindestens 36 French-Sonde, einer Resektion beginnend 2-5 cm vom Pylorus entfernt und 1 cm vom Ösophagus im His-Winkel endend mit einer Klammernahtverstärkung. LSG ist im Vergleich zu anderen Verfahren noch jung und zeigt exzellente Ergebnisse hinsichtlich Gewichtsverlust und Verbesserung von Komorbiditäten. Das Maximum an Gewichtsverlust wird 1-2 Jahre nach der OP erreicht und führt zu einem EWL von 50-60%. Langzeitdaten mit einem möglichen Wiederanstieg des Gewichtes und einer Wiederkehr von Erkrankungen fehlen noch immer.
