Zusammenfassung
Herr Sorgensen war sonntags mit seiner Frau immer im Schrebergarten, wenn es das Wetter zuließ. Sie waren froh, dass sie diese gemeinsame Zeit noch hatten, wobei Herr Sorgensen leicht einen anderen Eindruck vermittelte. „Diese Hitze und diese Kabel“, hörte man ihn leise vor sich hin fluchen. Frau Sorgensen schüttelte den Kopf und lächelte. Er war wieder der Alte. „Vergiss nicht, dass du froh sein kannst, nach den 5 Herzinfarkten wieder fluchen zu können! Ich habe nie gedacht, dass du noch einmal die Intensivstation verlassen würdest“. Den Antwortfluch konnte sie nicht mehr verstehen. Ihr Mann war im Geräteschuppen verschwunden und den Geräuschen nach räumte er ihn komplett um. In diesem Kapitel geht es um mechanische Herzunterstützungssysteme, Monitoring, Magnesium, Amiodaron, kardiale Kontraktilitätsmodulation, Ursachen von Anämien, die INTERMACS-Klassifikation, Antiarrhythmika, Entscheidungsfindung sowie Selbstkontrolle.

Die Richtungsbezeichnungen Inflow und Outflow beziehen sich auf die Flussrichtung in Relation zu der implantierten Pumpe und nicht auf die Flussrichtung in Relation zum Herz. Das ist manchmal etwas verwirrend.

Der wesentliche Unterschied bei der Reanimation von Patienten nach herzchirurgischen Eingriffen ist, dass bei frischoperierten Patienten (<10 d nach der Operation) der Thorax noch sehr instabil ist und häufige reversible Ursachen Perikardtamponade und Spannungspneumothorax sind. Deshalb empfehlen die Leitlinien bei insuffizienter Herzdruckmassage frühzeitig eine Notfallresternotomie durchzuführen. Unserer Meinung nach ist eine Übertragung dieser Empfehlung auf Patienten mit einem LVAD, der bereits vor längerer Zeit implantiert wurde, zumindest kritisch zu sehen. Eine Sternotomie durch einen in der Technik nicht Geübten erfordert viel Mut und wird kaum von Erfolg gekrönt sein.
