Zusammenfassung
Nur 1 % aller Kinder mit akuten Bauchschmerzen benötigt letztlich eine chirurgische Intervention. Diese Patienten sind als nicht nüchtern anzusehen. Wegen physiologischer Besonderheiten hat die klassische »Ileuseinleitung« keinen Platz in der Kinderanästhesie! Ein modifiziertes Vorgehen ist erforderlich mit dem Ziel einer sicheren intravenösen Narkoseeinleitung, einer optimalen Oxygenierung durch suffiziente Beatmung und einer atraumatischen Atemwegssicherung ohne Gegenwehr. Das perioperative Vorgehen konzentriert sich auf die konsequente Aufrechterhaltung der kindlichen Homöostase; eine suffiziente Organperfusion und Gewebeoxygenierung sind für die Narkoseführung essenziell. Hyponatriämie lässt sich durch den Einsatz balancierter Vollelektrolytlösungen mit einem möglichst physiologischen Natriumgehalt vermeiden. Bereits intraoperativ sollte das postoperative Management überdacht werden, um optimale Bedingungen für eine rasche Heilung zu schaffen.
