Zusammenfassung
Die atraumatische Femurkopfnekrose des Erwachsenen (aFKN) tritt im mittleren Alter auf. Risikofaktoren sind vor allem Steroide und Alkoholmissbrauch. Die frühe Diagnose ist wichtig für den Verlauf. Bei klinischem Verdacht wird geröntgt, bei unauffälligem Befund ist die MRT (ARCO-Klassifikation) Goldstandard. Ein Gelenkflächeneinbruch kann in der CT diagnostiziert werden. Unbehandelt kommt es in 2 Jahren zur subchondralen Fraktur. Die Gegenseite erkrankt innerhalb dieser Zeit, danach kaum. Konservativ können Iloprost oder Bisphosphonate als Heilversuch zum Einsatz kommen. Nur konservative Therapie (physikalisch) ist zur Therapie nicht geeignet. Die Entlastungsbohrung ist in frühen Stadien indiziert, ab ARCO IIIc oder IV die Hüft-TEP. Wichtige Differenzialdiagnose ist die transitorische Osteoporose, die im mittleren Alter sowie um das Ende der Schwangerschaft auftritt. Sie ist kein Frühstadium der aFKN. Die Diagnostik erfolgt mittels MRT, die Therapie konservativ (medikamentös, physikalisch).
