Zusammenfassung
Der Klumpfuß zählt zu den am stärksten funktionseinschränkenden Rückfußdeformitäten. Er ist durch typische Veränderungen der Stellung der Rück- und Vorfußgelenke zueinander charakterisiert. Die Diagnostik hat die einzelnen Bestandteile der Deformität durch klinische Tests und eine geeignete Bildgebung zu klassifizieren. Die Auswahl der Therapie richtet sich nach der Grunderkrankung sowie nach dem Alter der Patienten und der Ausprägung der Deformität. Es gibt eine Vielzahl verschiedener operativer Eingriffe an Vor- und Rückfuß, die zielorientiert auszuwählen und oft zu kombinieren sind. Die Beurteilung der intraoperativ erreichten Fußkorrektur sollte strikte Kriterien befolgen. Die postoperative Evaluation hat die Schwere des Ausgangsbefundes sowie die jeweiligen Auslösemechanismen zu berücksichtigen. Die Behandlung von postoperativen Rezidiven und Überkorrekturen stellt besonders hohe Anforderungen an den Chirurgen, der die bestmögliche Kompromisslösung suchen muss.
