Zusammenfassung
Die arthroskopische dorsale Stabilisierung der Schulter durch anatomische Refixation des abgerissenen Kapsel-Labrum-Komplexes an den Glenoidrand mit Fadenankern unter gleichzeitiger Raffung des posterioren inferioren glenohumeralen Bandes (pIGHL) ist ein Standardeingriff zur Behandlung der symptomatischen hinteren Schulterinstabilität. Bei in das Glenoid einhakenden reversen Hill-Sachs-Defekten sollte der Eingriff erweitert und je nach Größe des Defektes zusätzlich eine modifizierte McLaughlin-Operation oder eine Humeruskopfaugmentation durchgeführt und somit ein frühzeitiges Versagen der Operation verhindert werden. Auch wenn in der Literatur bisher nur einzelne Fallserien nach arthroskopischer hinterer Schulterstabilisierung publiziert sind, zeigt sich jedoch, dass nach genauer Analyse von Risikofaktoren und bei darauf abgestimmter Therapie sehr gute klinische Ergebnisse mit einer hohen Return-to-Sport-Rate zu erzielen sind.
