Zusammenfassung
Der Frakturfolgezustand Typ III beinhaltet als pathoanatomische Hauptkomponente die extrakapsuläre und dislozierte und pseudarthrotische Fehlstellung des Humeruskopfes gegenüber dem Humerusschaft auf Höhe des Collum chirurgicum humeri. Die Frakturdislokation im Collum chirurgicum beinhaltet mehrere Komponenten. Neben der Varus- oder Valgusverkippung und der anteroposterioren und mediolateralen Translation spielt vor allen Dingen auch die Rotation mit zunehmender Ante- oder Retroversion des Kopffragmentes eine Rolle. Im Gegensatz zu den übrigen Frakturfolgezuständen besteht bei intaktem Humeruskopf und intakter Rotatorenmanschette bei jüngeren Patienten die Option einer gelenkerhaltenden Osteosynthese mit Spongiosaplastik nach Pseudarthrosenresektion. Bei älteren Patienten spielt die inverse Schulterprothetik gegenüber der anatomischen Schulterprothetik eine immer bedeutendere Rolle, da sie am ehesten die Schulterfunktion wiederherzustellen vermag.
