Zusammenfassung

Nicht selten werden Patienten mit einer Herzklappenerkrankung erst im Rahmen der Prämedikationsvisite durch ein Herzgeräusch auffällig. Die häufigsten, perioperativ bedeutsamen Klappenerkrankungen sind die Aortenstenose und die Mitralinsuffizienz. Die Auswahl des Narkoseverfahrens bei Patienten mit Klappendefekt muss berücksichtigen, inwieweit die hämodynamischen Auswirkungen der Anästhesie (z. B. Änderungen von Herzfrequenz, Herzrhythmus, Blutdruck, peripherem und systemischem Gefäßwiderstand) mit der Pathophysiologie des Klappendefekts interferieren. Die meisten Vitien können medikamentös nur unzureichend behandelt werden. Ziel der Anästhesie ist daher die Erhaltung einer stabilen, nicht jedoch die Herstellung einer normalen Hämodynamik.
