Zusammenfassung

Vor einer Pharmakotherapie bedarf es möglichst zusammen mit Angehörigen einer Psychoedukation, d. h. Aufklärung über die Krankheit, die Behandlungsmöglichkeiten, die Wirklatenz der antidepressiven Wirkung, mögliche Nebenwirkungen. Dies ist Basis für die Therapieadhärenz angesichts hoher Noncompliance-Raten. Bei Verordnung und Auswahl sind je nach Substanz altersbedingte pharmakokinetische und pharmakodynamische Veränderungen (erforderliche Dosisanpassungen, Beachtung erhöhter Nebenwirkungsempfindlichkeit) zu beachten. Die antidepressive Pharmakotherapie sollte nach evidenzbasierten Kriterien mit einer niedrigen Anfangsdosis erfolgen („Start low, go slow“), dennoch ist auf eine ausreichend hohe Dosierung zu achten (TDM-Kontrolle). Zu den Substanzen 1. Wahl zählen Sertralin, Escitalopram und bei schweren Fällen Venlafaxin. Trizyklische Antidepressiva sollten auf Grund des Nebenwirkungsprofils nur in Ausnahmefällen zum Einsatz kommen. Angesichts häufiger Multimorbiditätist auf eine Limitierung der Polypharmazie zu achten, die Behandlung erfordert Kontrolluntersuchungen und eine gute Kooperation und Abstimmung insbesondere mit den hausärztlichen Kollegen.
