Zusammenfassung

Diagnose und Therapie psychiatrischer Notfälle und Krisen gehören wegen deren Häufigkeit und Bedeutung zum unverzichtbaren Grundlagenwissen. Die Begriffe „Notfall“ und „Krise“ sind nicht scharf voneinander zu trennen. Ein psychiatrischer Notfall ist ein krankheitsbedingter Zustand, der zu unmittelbarem Handeln zwingt. Er erfordert eine sofortige, an der akuten Symptomatik orientierte gezielte Therapie, um Gefahr für die Gesundheit des Patienten und ggf. anderer Personen abzuwenden. Die wichtigsten psychiatrischen Notfälle sind: akute Suizidalität, akute Unruhe- und Erregungszustände, Angststörungen, Bewusstseinsstörungen, delirante Syndrome/Verwirrtheitszustände, Stupor und Katatonie, Intoxikationen und andere substanzbedingte Störungen. Die psychiatrische Krise ist weniger durch direkte vitale Bedrohung gekennzeichnet als durch das Fehlen oder das Zusammenbrechen individueller und/oder sozialer Bewältigungsstrategien im Rahmen belastender Krankheits- bzw. Umgebungsbedingungen. Die psychiatrische Krisenintervention ist bestrebt, in mehreren Schritten innerhalb von Tagen oder Wochen eine ursächliche Veränderung der zugrunde liegenden Bedingungen zu erreichen. Dazu ist in größerem Ausmaß als beim psychiatrischen Notfall und in Abgrenzung zur „normalen Krise“ auch nichtärztliche professionelle Hilfe erforderlich.
