Zusammenfassung

Schizophrene Psychosen sind gekennzeichnet durch charakteristische, symptomatisch oft sehr vielgestaltige psychopathologische Querschnittsbilder mit Wahn, Halluzinationen, formalen Denkstörungen, Ich-Störungen, Affektstörungen und psychomotorischen Störungen. Der Verlauf ist heterogen und reicht von einer vollständigen Remission der Symptome bis zu einem chronischen Residualsyndrom. Die meisten Patienten erleben rezidivierende Erkrankungsepisoden. Die Ätiologie der Schizophrenie gilt als multifaktoriell. Es liegen heute vielfältige Befunde zu biologischen und psychosozialen Auffälligkeiten vor, wobei häufig unklar ist, ob diese die Störung mitbegründen oder ein Ergebnis ihres Verlaufs sind, am ehesten beides. In der Akutphase, der Postakutphase sowie zur Rezidivprophylaxe einer schizophrenen Psychose kommen antipsychotisch wirksame Medikamente zum Einsatz, die eine wichtige Säule der Therapie darstellen. Zusätzlich sollten alle Patienten im Rahmen ihrer Erkrankung eine kognitive Verhaltenstherapie erhalten. Neuere psychotherapeutische und psychosoziale Interventionen haben zuletzt weitere Evidenz für die Wirksamkeit gewonnen und spielen von Anfang an bei der Symptombehandlung als auch im weiteren Verlauf bei der Krankheitsverarbeitung sowie beim Erhalt des Funktionsniveaus als zusätzliche therapeutische Säule eine wesentliche Rolle. Neurostimulationsverfahren wie die Elektrokonvulsionsbehandlung oder neuere Stimulationsverfahren werden v. a. in therapieresistenten Fällen eingesetzt.
